舒芬太尼复合丙泊酚靶控输注麻醉对老年患者意识和苏醒的影响

摘　　要：目的探讨舒芬太尼复合丙泊酚靶控输注(target controlled infusion,TCI)麻醉对老年患者意识和苏醒的影响。方法收集我院外科老年上腹部手术患者80例,按照随机数字表法分为试验组和对照组各40例,试验组给予舒芬太尼复合丙泊酚TCI麻醉,对照组给予静吸复合麻醉,观察两组自主呼吸时间、呼唤睁眼时间、刺激反应恢复时间、拔管时间、苏醒评分等指标。结果试验组自主呼吸时间、呼唤睁眼时间、刺激反应恢复时间以及拔管时间均较对照组短,差异有统计学意义(P<0.05),试验组麻醉Steward苏醒评分高于对照组,差异有统计学意义(P<0.05)。术后24 h两组意识评分和认知评分均较术前明显降低(P<0.05),但两组间差异无统计学意义(P>0.05)。结论舒芬太尼复合丙泊酚TCI麻醉可以明显缩短患者苏醒的时间,对老年患者意识和认知的影响较轻,不良反应发生情况较少,值得临床上推广应用。Objective To investigate the effects of sufentanil and propofol target controlled infusion （TCI）anesthesia on con- sciousness and awakening in elderly patients. Methods Eighty elderly patients who underwent upper abdominal operation were ran- domly divided into experimental and control groups,40 cases in each group. The experimental group received a sufentanil and propofol target controlled infusion anesthesia while the control group was given an intravenous inhalation anesthesia. The impact of the treatments on state of consciousness and awakening were observed. Results Spontaneous breathing time, time of calling eye opening, time of stim- ulation response recovery and time of extubation time in the experimental group were all significantly shorter than that in the control group （ P 〈 0.05 ）. Steward awake anesthesia score in the experimental group was higher than that in the control group （ P 〈 0. 05 ）. Postoperative 24 hr consciousness and cognitive scores were significantly reduced when compared to before anesthesia （P 〈 0. 05 ）, but the difference between the two groups was not statistically significant. Conclusion Sufentanil target controlled infusion of propofol an- esthesia can significantly reduce the patient awake time, and has a less impact on consciousness and cognition and less adverse reactions in elderly patients. It is worth for clinical adoption.
