摘 要:目的探讨人乳头瘤病毒(HR-HPV,HPV6、11)、单纯疱疹病毒II型(HSV—Ⅱ)感染及其-9宫颈癌前病变的关系。方法收集1876份宫颈细胞学样本,采用实时荧光PCR技术测定HR—HPV,HSV-Ⅱ和HPV6、11的核酸,并对其宫颈组织进行细胞学检测。结果18—25岁、26—35岁、36—55岁组HPV6、11感染率分别为7.68%、4.09%和2.94%,HR—HPV感染率分别为11.61%、11.78%和9.96%,HSV—Ⅱ感染率分别为3.53%、3.11%和3.19%。不同程度宫颈病变中HR—HPV感染率分别为:未出现异常细胞(NILM)4.19%(68/1621),不典型鳞状细胞(ASCUS)55.85%(64/115),鳞状上皮内低度病变(LSIL)83.99%(42/50),鳞状上皮内高度病变(HSIL)92.85%(39/42),鳞状细胞癌(SCC)100%(3/3)。宫颈细胞学异常组HR—HPV与HPV6、11合并感染率显著高于正常组(P〈0.05)。HPV6、11感染率随着年龄增高而下降,HR—HPV感染率-9宫颈病变严重程度呈正相关(P〈0.05)。结论HPV6、11感染率存在年龄差异性,HR—HPV和HPV6、11混合感染更易导致宫颈癌前病蛮.Objective To investigate the relationship between precancerous lesions in uterine cervix and infection of human papilloma virus ( HR-HPV, HPV6 and HPV11 ) or herpes simplex virus type 2 (HSV-Ⅱ). Methods A total of 1876 of cervix cell speci- mens were collected. The infection of HR-HPV, HSV-Ⅱ, HPV6 and HPV11 was determined by using a real time PCR. At the same time, cytological examination was used to assess the cervical lesions. Results The infection rates of HPV6 and HPV11 in three age groups (18 to 25,26 ~ 35 and 36 ~ 55 years old) were 7.68% ,4. 09% and 2. 94% , respectively. The infection rates of HR-HPV were 11.61% ,11.78% and 11.61% ,respectively. The HSV-II infection rates were 3.53% ,3.11% and 3.19,respectively. The HR-HPV infection rates in different degree of cervix lesions were 4. 19% (68/1621) in no abnormal cells appear(NILM) group,55.85% (64/ 115 ) in atypical squamous cells (ASCUS) group, 83.99 % ( 42/50 ) in squamous intraepithelial low-grade lesions (LSIL) group, 92. 85 % ( 39/42 ) in seriously squamous intraepithelial lesion (HSIL) group and 100% ( 3/3 ) in squamous cell carcinoma ( SCC ) group, respec- tively. The co-infection rate of HR-HPV with HPV6 and/or HPV11 in cytological abnormal group was significantly higher than that in cytological normal group(P 〈 0. 05). Conclusion There is a difference in infection rate of HPV6 and HPVll among ages. Co-infec- tion of HR-HPV and HPV6 or HPV11 is more likely to cause cervical precancerous lesions.
