儿童布氏杆菌病12例临床特点分析

摘　　要：目的研究儿童布氏杆菌病的特点,减少误诊。方法观察并总结儿童布氏杆菌病患儿的临床特点、血常规特点及误诊情况,并与同期住院的成人布氏杆菌病患者进行对比。结果儿童布氏杆菌病主要表现为发热(100.00%)、大汗(58.33%)、咳嗽(58.33%)、关节痛(25.00%)。儿童脾大者较成人少(P=0.04)。外周血白细胞正常或轻度下降,淋巴细胞比例升高者占83.33%,单核细胞比例升高者占66.67%,少数有贫血(25.00%),血小板减少者未见。和成人相比,单核细胞升高者多(P=0.04),血小板减少者少(P=0.04)。儿童布氏杆菌病能够三日确诊者为0,误诊率达到100%,平均确诊时间为(24.33±3.86)d;与成人患者比,三日确诊例数少(P=0.06),更容易误诊(P=0.00),确诊时间更长(P<0.01)。结论儿童布氏杆菌病患儿流行病学史隐匿,表现复杂多样,容易误诊,需引起高度重视。Objective To study the characteristics of brucellosis in children and reduce misdiagnosis. Methods To observe and summarize the clinical characteristics of children with children＇s brucellosis, blood routine characteristics and misdiagnosis. And adult brinell coli in hospital and the patients were compared. Results There was 100.00% brucellosis mainly for children fever, sweating（58.33%）, cough（58.33%）, joint pain（25.00%）. And adults were less than children spleen supplied（ P =0.04）. Peripheral blood leucocyte normal or slightly decreased, lymphocytes increased83.33%, monocyte proportion increased by 66.67%, a few 25.00% had anemia, thrombocytopenia was not seen. Compared with adults increased monocyte more（P=0.04） and thrombocytopenia was less（P=0.04）. Children brucellosis to those diagnosed in the three days of 0, the misdiagnosis rate of 100%, the average confirmed time was（24.33 ±3.86）d.And adult patients, 3 confirmed cases less（P=0.06）, more easily misdiagnosed（P=0.00）, confirmed longer（P〈0.01）.Conclusion Epidemiological history of brucellosis in children with hidden, complex and varied performance, easy misdiagnosis, attaches great importance to it.
