结石性脓肾临床分析

摘　　要：目的 探讨结石性脓肾的诊断及治疗措施.方法 对2010年6月-2013年6月新疆维吾尔自治区人民医院泌尿外科经实验室及影像学检查确诊的60例结石性脓肾患者的病历资料进行回顾性的临床分析.19例患者行Ⅰ期经皮肾镜取石术,20例行输尿管镜下输尿管结石碎石术,15例行经皮肾穿刺造瘘引流术,6例患者行经输尿管镜支架置入术.结果 60例患者中,42例（70％）肾功能得以不同程度的恢复,Ⅰ期结石取净者31例,残留碎石29例,二期结石取净者18例,三期结石取净者10例,1例Ⅱ期肾切除术.术后随访1-l2个月无复发.结论 微创腔镜技术对结石性脓肾进行治疗效果良好,个体化治疗结石性脓肾,大大提高手术成功率.Objectives To investigate the diagnosis and treatment of the Calulus pyonephrosis.Methods The clinical data of 60 patients of calculous pyonephrosis were studied for the analysis from June,2010 to June,2013.Nineteen cases were treated by one-stage pereutaneous nephrolithotomy,20 cases by transurethral ureteroscopy,15 cases by mini-percutaneous nephrostomy,nephrollithtomy,6 cases by Double-J ureteral stenting with ureterseop.Results Renal function recovered to different degree in 42 out of 60 cases（70％）,The stone free underwent one-stage operation were 31,29 cases had residual stones.The stone free underwent second-stage operation were 18,third-stage operation were 10,1 case second-stage nephrectomy.No other complications occurred after follow-up of 1 to 12 months.Conclusions Minimally invasive endoscopic technique on calculous pyonephrosis is good treatment.The surgical treatment should be based on the patients＇situation,greatly affecting the success rate of operation.
