压力容积导管系统评价心梗模型心功能的意义

摘　　要：目的应用压力容积导管系统对心肌梗死大鼠进行左心室血液动力学检测。方法采用结扎左冠状动脉前降支的方法制备心肌梗死模型作为模型组,行假手术作为假手术组。2周后,分别对2组大鼠进行左心功能进行检测,大鼠麻醉后经颈动脉向左心室插入Millar导管,记录大鼠左心室的血流动力学参数值,然后进行分析。结果模型组在反应左心室排血功能的参数SW(P<0.01)、CO(P<0.01)、SV(P<0.01)、Pes(P<0.01)、EF(P<0.01)、Pow Max(P<0.01)、-d V/dt min(P<0.01)均降低和Ves(P<0.01)增加;模型组在反应左心室舒张功能的参数Ved(P<0.01)、Ped(P<0.01)均升高和-d P/dtmin(P<0.05)、+d V/dtmax(P<0.01)均降低。结论 2周后心肌梗死模型组大鼠左心室主动舒张功能无障碍,但是左心室收缩功能降低,可见压力容积导管系统可以客观准确的评价大鼠的心功能变化。Objective：To test the left ventricular hemodynamics of myocardial infarction rats by the use of pressurevolume catheter system. Methods ： A myocardial infarction model was made by the ligation of left anterior descending coronary artery and sham surgery used as a sham surgery group. The left ventrieular function of the two groups were detected two weeks later and the left ventricular hemodynamic parameters were recorded and analyzed after the rats were anesthetized by inserting Millar catheter into the left ventricle through carotid artery. Results：The left ventricular ejection function parameters of the model group,SW（P 〈0.01 ） ,CO（P 〈0.01 ） ,SV（P 〈0.01 ） ,Pes（ P 〈0.01 ） ,EF（P 〈0.01 ） ,PowMax（P 〈0.01 ）, - dV/dt min（P 〈0.01 ）lowered while Ves（P 〈0.01 ） increased. The left ventricular diastolic function parameters of the model group, Ved （ P 〈 0.01 ） and Ped （ P 〈 0.01 ） rose while - dP / dtmin （ P 〈 0.05 ） and ＋ dV / dtmax （ P 〈 0. 01 ）lowered. Conclusion：The left ventricular diastolic function of the myocardial infarction rat model group was accessible two weeks later,but the left ventricular systolic function reduced. It can be seen the pressure -volume catheter system can accurately evaluate the changes of rat heart function objectively.
