腰俞穴麻醉复合针刺麻醉对微创肛肠手术临床并发症的影响

摘　　要：目的：观察腰俞穴麻醉复合针刺麻醉对微创肛肠手术临床并发症的影响。方法：选择我院2012年至2013年收治的择期肛肠手术患者60例,按随机数字表将其分为试验组和对照组,每组30例。试验组患者在腰俞穴麻醉前采用韩式神经刺激仪予以经皮穴位电刺激,选择双侧＂内关＂,＂足三里＂,＂合谷＂,＂三阴交＂等穴位,刺激频率为2/100Hz,强度2~8m A以患者最大承受为度。于术前30 min予以经皮穴位电刺激,刺激时间为30 min,每2次刺激间隔15 min,持续至手术结束。30 min后行腰俞穴麻醉。对照组只行腰俞穴麻醉,不予以穴位刺激。记录两组患者入室后（T0）、行针刺麻醉前（T1）、针刺麻醉后（T2）、吻合器吻合前（T3）、牵吻合器吻合后（T4）、手术结束后（T5）的生命体征,包括平均动脉压（MAP）、心率（HR）、血氧饱和度（SPO2）;同时记录患者牵拉反射、术后舒适度、麻醉不良反应及安置尿管率比较。结果：两组患者在T1、T2、T3、T4 T5各个时点,HR、MAP、RR、SPO2意义相比差异无统计学意义（P〉0.05）,与对照组相比,试验组患者在术中牵拉反射、术后舒适度、麻醉不良反应及安置尿管的例数方面差异有统计学意义（P〈0.05）。结论：腰俞穴麻醉复合针刺麻醉应用在肛肠手术临床并发症中安全性、舒适度方面明显优于单纯腰俞穴麻醉。Objective：To observe the effect of combination of yaoshu point anesthesia and acupuncture anesthesia on complications in minimally invasive anorectal surgery. Methods： Sixty inpatients undergoing selected anorectal surgery in 2012 ~2013 were randomly divided into test group and control group,30 inpatients each. Combination of yaoshu point anesthesia and acupuncture anesthesia was performed in test group,while simple yaoshu point anesthesia was used in control group.In test group,transcutaneous acupuncture electrical stimulation（TAES） was administerd before yaoshu point anesthesia at acupoints of bilateral P6, ST 36, LI 4 and SP6（for 30 min,2/100 Hz,2~8 m A）.The vital signs,including mean arterial pressure（MAP）, heart rate（HR） and SPO2,as well as the stretch reflex, postoperation comfort,anesthesia adverse reaction and the rate of catheter placement were recorded in both groups at 5 different time points. Results：There were no differences in HR,MAP,SPO2 but there were significant differences in stretch reflex,postoperation comfort,anesthesia adverse reaction and the catheter placement rate between two groups. Conclusion： Combination of yaoshu point anesthesia and acupuncture anesthesia is better than simple yaoshu point anesthesia in clinic safety and comfort in anorectal surgery.
