补肾活血方对膝骨性关节炎的临床观察

摘　　要：目的:观察补肾活血方治疗膝骨性关节炎的临床疗效。方法:将符合纳入标准的71例患者,采用随机对照法,分为治疗组35例和对照组36例。治疗组给予口服补肾活血方,对照组给予口服西乐葆,两组均以12周为观察期,对两组患者治疗前后肿胀、疼痛、Lysholms评分、临床主要症状、中医伴随症状、安全性进行评价。结果:治疗组总有效率97.14%,对照组总有效率94.44%,两组总有效率无明显性差异(P>0.05);与治疗前比较,治疗组和对照组治疗前后各症状、功能积分之间(临床症状、肿胀程度、疼痛程度、膝关节功能)均有显著性差异(P<0.001)。组间比较,两组治疗膝骨性关节炎在临床主要症状表现以及早期改善膝关节功能方面具有统计学意义(P<0.05),在中医伴随症状中,两组在缓解腰膝酸软、改善耳鸣等方面具有统计学意义(P<0.05),且治疗组优于对照组;两组在治疗膝骨关节炎疼痛程度、肿胀程度、膝关节功能方面效果相当(P>0.05)。结论:补肾活血方能明显改善膝骨性关节炎患者的症状及体征,在临床主要症状表现、早期改善膝关节功能以及中医伴随症状(如腰膝酸软、耳鸣等)方面,疗效优于西乐葆。Objective. To observe the clinical therapeutic effect of Bushen Huoxue Decoction in the treatment of osteoarthritis of knee joint. Methods ：71 patients who met the inclusion criteria were randomized into two groups ,35 patients in treatment group and control group with 36 cases. The treatment group was given oral Bushen Huoxue Decoction and the control geouP was treated with oral administration of Celebrex. 12 weeks was the observation period for the two groups. Before and after treatment ,swelling ,pain, Lysholms score, clinical symptoms, TCM symptoms and safety evaluation were compared. Results ： The total effective rate of treat- ment group was 97. 14% and control group＇s total efficiency was 94. 44%. Two groups had no significant difference （P 〉 0. 05 ）. Compared with before treatment,the treatment group and the control group＇s integral various symptoms and functional scores after treatment （the clinical symptoms, the degree of swelling, pain, knee joint function） had significant difference （P 〈 0. 001 ）. Two groups＇ knee osteoarthritis in the main clinical symptoms and early improvement of knee joint function was statistically signif- icant （ P 〈 0. 05 ）. In the traditional Chinese medicine symptoms, two groups had statistical significance in alleviating the lumbar and knees weakness and improving tinnitus （P 〈 0. 05） and the treatment group was better than the control group. Two groups in the treatment of the knee pain, swelling, the knee joint function were equivalent （P 〉 O. 05）. Conclusion ： Bushen Huoxue Decoc- tion can obviously improve the symptoms and signs of patients with knee osteoarthritis, clinical symptoms, mainly in the early im- proving knee function and TCM symptoms such as soreness and weakness of waist and knees, tinnitus, and the curative effect is better than celebrex.
