摘 要:目的探讨医院肺炎克雷伯菌耐药率与抗菌药物使用强度的相关性,为临床合理使用抗菌药物提供依据。方法统计医院2012年检出的252株肺炎克雷伯菌的耐药率,结合同期抗菌药物使用强度,分析两者相关性;分析肺炎克雷伯菌对不同类别抗菌药物耐药率的差异。结果肺炎克雷伯菌的耐药率与抗菌药物的使用强度呈极显著线性正相关关系;对不同类别抗菌药物的耐药率不尽相同,对氨基苷类和喹诺酮类的耐药率差异无显著性,对广谱青霉素类的耐药率最高,对碳青霉烯类、含酶抑制剂头孢类的耐药率最低。结论肺炎克雷伯菌耐药率与抗菌药物的使用强度有关,临床可酌情优选含酶抑制剂头孢类、氨基苷类、喹诺酮类、碳青霉烯类治疗,暂停选用广谱青霉素类。Objective To investigate the correlation between the drug-resistant rates of Klebsiella pneumoniae(Kp) and antibacterial use density(AUD) to provide the basis for the reasonable use of antibacterial drugs in clinic. Methods The drug-resistant rates of Kp in de- tected 252 strains of Kp during 2012 were summarized and statistically analyzed,which was combined with contemporaneous AUD for ana- lyzing the correlation between them;the difference of resistant rates of Kp to different categories of antibacterial drugs was statistically ana- lyzed. Results The drug- resistant rate of Kp exhibited extremely significant positive correlation with AUD. The resistant rates of Kp to dif- ferent categories of antibacterial drugs were different,the difference of resistant rate to aminoglycosides and fluoroquinolones had no statistical significance,the resistant rate to broad spectrum penicilfins was highest and which to carbapenems and cephalosporins containing enzyme in- hibitors was lowest. Conclusion The drug- resistant rate of Kp is correlated with AUD. Clinic may give preference to cephalosporins con- taining enzyme inhibitors,aminoglycosides,fluoroquinolones and carbapenems and suspends the use of broad-spectrum penicillins.
