护理套件预防经外周静脉置入中心静脉导管相关感染的效果

摘　　要：目的探讨护理套件在预防经外周静脉置入中心静脉导管(peripherally inserted central catheter,PICC)相关感染中的应用效果。方法选取2010年1月至2012年12月在上海市第一人民医院行PICC置管术的患者共1745例,将2010年1月至2011年1月收治的687例患者作为非护理套件组,将2011年2月(我院开始使用中心静脉置管术护理套件进行PICC导管的维护)至2012年12月收治的患者1058例作为护理套件组,两组患者均按照标准进行PICC置管及维护,分析并比较两组患者导管穿刺部位感染和导管相关性血流感染(catheter-related blood stream infection,CRBSI)的发生率。结果使用护理套件组患者的导管穿刺部位感染和导管相关血流感染的发生率均低于非护理套件组,差异均有统计学意义(均P<0.05)。结论使用统一的护理套件可减少PICC患者CRBSI及导管穿刺部位感染的发生。Objective To discuss the application effect of nursing kits in PICC for the prevention of cathe-ter-related infections.Methods A total of 1,745 patients who had PICCs were included.687 patients which admission from January 2010 to January 201 1 were selected as non nursing kits group,and 1058 patients which admission for February 201 1 to December 2012 were selected as nursing kits group.Two groups of patients had PICC catheter maintenance according to standard.The catheter-related bloodstream infections and catheter insertion site infections were compared between the two groups.Results The group which used nursing kits had lower incidence rate of catheter-related bloodstream infections and catheter insertion site infections than non nursing kit group（all P 〈0.05）.Conclusion The use of standardized nursing kits can effectively reduce the incidence rate of catheter-related bloodstream infections and catheter insertion site infections in PICC.
