医院门诊药房处方差错的原因分析及其对策

摘　　要：目的 探讨医院门诊药房处方差错的原因及对策.方法 对2011-2013年杭州市萧山区第一人民医院门诊药房2 180份处方进行回顾性分析,按照干预措施实施与否分为非干预组（1 102份）与干预组（1 078份）.比较2组诊药房处方差错发生情况.结果 药物品种差错、药物数量差错、剂量剂型差错、用法用量差错、包装相似差错、注射单填写差错、错发患者差错、医师处方差错及收费差错发生率,非干预组分别为2.0％（22/1 102）、3.0％（33/1 102）、2.5％（28/1 102）、3.3％（36/1 102）、1.8％（20/1 102）、1.5％（16/1 102）、1.3％（14/1 102）、0.9％（10/1 102）及1.4％ （15/1 102）,干预组分别为0.8％ （9/1 078）、0.7％（7/1 078）、0.7％（8/1 078）、1.0％（11/1 078）、0.6％ （6/1 078）、0.3％ （3/1 078）、0.6％ （6/1 078）、0.2％ （2/1 078）及0.4％（4/1 078）,2组比较差异有统计学意义（P＜0.05）.结论 采取积极的、正确的门诊药房处方差错干预对策,能够明显改善医院门诊药房的调剂质量及患者生活质量.Objective To explore the common causes of hospital outpatient pharmacy prescription errors and specific countermeasures.Methods Patients diagnosed in Xiao Shan first People＆#39;s Hospital of Hangzhou City were selected from 2012 to 2013.2 180 hospital outpatient pharmacy prescriptions were retrospectively analyzed,according to whether or not to implement interventions divided into the non-intervention group （n =1 102） and the intervention group （n =1 078）.pharmacy prescription errors in both compared.Results The errors of drugs varieties,drug number,dosage forms,usage and dosage,similar packaging,injection filling,sent to the wrong patient,prescription and charges were significantly higher in non-intervention group than in the intervention group [2.0% （22/1 102）,3.0%（33/1 102）,2.5%（28/1 102）,3.3%（36/1 102）,1.8%（20/1 102）,1.5%（16/ 1 102）,1.3% （14/1 102）,0.9% （10/1 102）and 1.4% （15/1 102）,respectively,in non-intervention group,0.8% （9/1 078）,0.7% （7/1 078）,0.7% （8/1 078）,1.0% （11/1 078）,0.6% （6/1 078）,0.3% （3/1 078）,0.6%（6/1078）,0.2%（2/1078）and0.4%（4/1 078）intheinterventiongroup,P〈0.05] Conclusion Taking intervention measures and positively correcting errors in the outpatient pharmacy prescriptions,can significantly improve the quality in patients swap hospital outpatient pharmacies.Conclusion Take positive and correct errors in the outpatient pharmacy prescriptions intervention measures,can significantly improve the quality and quality of life in patients swap hospital outpatient pharmacies.
