临时心脏起搏器术后患者心脏并发症护理对策

摘　　要：目的：探讨置入临时心脏起搏器患者心脏并发症及相应护理对策。方法入选2011年11月至2013年11月于解放军白求恩国际和平医院心血管内科接受临时心脏起搏器置入术患者106例。其中男性89例，女性17例，年龄40～83岁，平均年龄（65.6±4.9）岁。针对术后可能出现的并发症，提出并实施相应护理措施。结果发生心律失常20例，因起搏电极刺激或移位导致10例室性早搏，8例室性心动过速，2例室颤。心室穿孔2例，患者均为急性下壁右室心梗，调整临时起搏电极位置时损伤局部心肌而致。经对症处理后患者均症状好转出院。结论术后对并发症密切观察，实施相应护理措施，有助于改善患者预后。Objective To discuss the nursing countermeasures for patients with postoperative cardiac complications after temporary cardiac pacemaker implantation. Methods The patients （n=106） were chosen from the Department of Cardiovasology of PLA Bethune International Peace Hospital from Nov. 2011 to Nov.2013, including 89 male patients and 17 female ones. Their age was from 40 to 83 and average age was （65.6±4.9）. The corresponding nursing countermeasures were proposed and carried out aiming at possible complications. Results There were 20 patients with arrhythmia, 10 with ventricular premature beat due to pacemaker electrode stimulation or shift, 8 with ventricular tachycardia, 2 with ventrieular fibrillation and 2 with ventricular perforation due to local myocardial injury by adjusting pacemaker electrode position because of acute right inferior myocardial infarction. All patients were better and discharged from the hospital after symptomatic treatment. Conclusion Close observation and corresponding nursing countermeasures to postoperative cardiac complications will improve patients ＇ prognosis.
