摘　　要：目的调查综合医院肺炎患者嗜肺军团菌的感染现况。方法于2012年6月—2013年6月采集北京市丰台区、常州市、无锡市、深圳市共7家综合医院279例肺炎住院患者的痰、尿和血清,采用巢氏PCR法检测痰中嗜肺军团菌,酶联免疫吸附法检测尿中嗜肺军团菌抗原和血清嗜肺军团菌抗体。结果痰嗜肺军团菌、尿嗜肺军团菌抗原和血清嗜肺军团菌抗体阳性率分别为10.4%(19/182),9.7%(22/226)和20.4%(46/226)。综合医院肺炎患者嗜肺军团菌感染率为27.6%(77/279),其中丰台、无锡、常州和深圳四地区感染率分别为14.7%(14/95),39.6%(19/48),32.7%(18/55)和32.1%(26/81),差异有统计学意义(χ2=12.860,P=0.005)。结论本次调查的综合医院肺炎患者中存在嗜肺军团菌感染。Objective To investigate Legionella pneumophila infection status in patients with pneumonia from general hospitals.Methods The sputum, urine and serum samples were collected from 279 patients with pneumonia in general hospitals of four cities during Jun., 2012 to Jun., 2013. Legionella pneumophila in sputum was detected by nested-PCR. Legionella pneumophila antigen in urine and Legionella pneumophila antibody in serum were detected by ELISA. Results The positive rate of Legionella pneumophila in sputum, Legionella pneumophila antigen in urine and Legionella pneumophila antibody in serum were 10.4%（19/182）, 9.7%（22/226） and 20.4%（46/226） respectively. The Legionella pneumophila infection rate of patients with pneumonia was 27.6%（77/279）. The Legionella pneumophila infection rate of patients with pneumonia in general hospitals of Fengtai, Wuxi, Changzhou and Shenzhen were 14.7%（14/95）, 39.6%（19/48）, 32.7%（18/55） and 32.1%（26/81） respectively. There was significant difference in infection rates of Legionella pneumophila among four investigatied cities（χ2=12.860, P =0.005）.Conclusion Legionella pneumophila infection was found in patients with pneumonia in the investigated general hospitals.
