摘  要:目的:通过椎体压缩骨折外伤时间与MRI表现的相关性研究,探讨MRI对比信噪比(contrast noise ratio,CNR)在推断外伤性椎体压缩骨折时间中的应用价值。方法:选取外伤性胸、腰椎压缩骨折患者76例及志愿者20例。根据外伤时间(T),将76例分为T<7d、7d≤T<15d、15d≤T<30d、30d≤T<60d、60d≤T<90d、90d≤T<120d共6组。所有患者应用脊柱线圈TSE序列,常规行矢状位T2WI、T1WI、SPAIR扫描,轴位T2WI扫描,观察各时段骨折部位信号特点,测量信号强度,计算CNR。结果:椎体骨折部位T1WI、T2WI信号强度及SNR与骨折时间无相关性。而骨折部位SPAIR信号强度及CNR与骨折时间均具有相关性(P<0.05),其中SPAIR的CNR与骨折时间的相关性最强(P<0.01)。随着骨折愈合时间的延长,SPAIR的CNR逐渐降低,骨折≥60d患者SPAIR的CNR与对照组比较差异无统计意义(P>0.05)。结论:运用MRI的量化指标CNR可推测外伤性胸、腰椎压缩骨折损伤时间,其中SPAIR的CNR与骨折时间的相关性最强。Objective: To explore the value of MRI contrast noise ratio (CNR) in evaluation of injury time of traumatic thoracic and lumbar vertebral compression fractures. Methods: 76 cases of traumatic thoracic and lumbar vertebral compression fractures patients and 20 volunteers were chosen. 76 patients were divided into 6 groups according to fracture injury time. TSE T2 WI,T1WI and SPAIR were performed and the signal intensity was determined. CNR values were calculated. Results: There was no correlation between T2 WI, TI WI signal intensity, CNR and the fracture time. There was an obvious correlation between SPAIR signal intensity, CNR and the fracture time. SPAIR CNR gradually declined during healing, SPAIR CNR in patients with fracture of 60+ d had no significant difference compa ring with volunteers (P〈0.05). Conclusions.. Using MRI quantitative indicators can speculate on traumatic thoracic and lumbar spinal compression fracture injury time,and SPAIR CNR has relevance to fracture time.
