摘  要:目的探讨甲胎蛋白(alpha-fetoprotein,AFP)、癌胚抗原(carcinoembryonic antigen,CEA)在肝癌诊断中的应用价值。方法选择原发性肝癌(primary hepatic cancer,PHC)组71例,转移性肝癌(metastatic hepatic carcinoma,MHC)组27例,健康体检对照(normal control,NC)组54例,采用电化学发光免疫检测各组AFP、CEA,并对结果进行统计学分析。结果 PHC组AFP水平明显高于MHC组和NC组(P<0.001),AFP在PHC组中检出的敏感率为78.87%;MHC组中CEA的表达为(248.19±54.16)ng/m L明显高于PHC组和NC组(P<0.001),CEA在MHC组中检出的敏感率为55.55%;AFP、CEA诊断原发性肝癌ROC曲线下面积分别为0.874、0.583。结论检测AFP、CEA在肝癌的诊断与鉴别诊断中具有较高的临床价值。Objective To evaluate the clinical value of the alpha-fetoprotein( AFP),carcinoembryonic antigen( CEA) in diagnosis and differential diagnosis of hepatocellular carcinoma.Methods Serum AFP,CEA levels were detected by electrochemical luminescence immunoassay in71 patients with primary liver cancer( PHC),27 patients with metastatic liver cancer( MHC) and 54 healthy control subjects. Results The level of AFP in PHC group was significantly higher than that in MHC and normal control( NC) groups( P < 0. 001),the sensitive rate of AFP detection in PHC group was 78. 87%. In MHC group,the CEA expression level( 248. 19 ± 54. 16) ng / m L was significantly higher than that in PHC and NC groups( P < 0. 001). The sensitive rate of CEA detection in the MHC group was 55. 55%. The area under the curve( AUC) of the AFP and CEA were 0. 874 and 0. 583 respectively. Conclusion The clinical value of detection of AFP,CEA was higher in the diagnosis and differential diagnosis of hepatocellular carcinoma.
