农村社区传染病健康管理模式的初步研究

摘　　要：目的:探讨农村社区传染病健康管理(Health management infections diseases,HMID)模式。方法:采取查阅资料、实地调查、访谈、对比分析方法。选择3个省的行政和医务人员、教师和患者等240余人进行调查和访谈。结果:传染病仍是危害农村社区的公共卫生问题。HMID的理念和实施方案缺乏;现行传染病疫情网络直报系统难以识别病人流动状态;传染病健康知识教育有待改进;基层传染病防控队伍未发挥好防控第一关口作用;财政投入方向有待探讨。结论:传染病防治是社会问题的观念是改变现状和制度建设的保障,设计了HMID模式的体系框架,把传染病知识和道德教育作为中小学素质教育内容是基础措施,编制"慢性传染病单病健康知识手册"是调动管理对象自觉性和主动性的办法,利用现代技术为传染病患者创建网上家园是重要补充;将公安系统和出入境检疫机构管理的流动人口信息实时传递到同级疾控中心是监测的有效措施;农村社区卫生机构是实施HMID的主体;优化财政投入方向是政策推手。Objective To study the mode of health management of infectious diseases（HMID）in rural communities.Methods The spot investigation,interview and contrast analysis were performed in more than 240 administrative,medical staff,teachers and patients in three provinces.Results Infectious disease（ID）was still an important public health problem in rural community.The concept of HMID had not been popularized because of lacking measures.The current situation of direct report network system of IDs could not identify the shift of patients.The knowledge of IDs should be promoted.The control quality of ID at the grass-roots level did not play a key role in the first step of prevention.The financial investment needed to be discussed.Conclusions ID is a social problem and the framework of HMID should be constructed.The knowledge and moral education of ID for elementary and middle schools education is the basis for the HMID.Compiling ＂ Manual of Health Knowledge of Single Chronic IDs＂ is the effective way to mobilize the initiative of patients,and using modern technology to create online home for the patients is an important supplement.Making the information of flowing population from the public security and entry/exit inspection transferred to the centers of disease control in real-time is an effective monitoring approach.The health service institutions in rural communities constitute the main body of HMID.The optimization of financial investment is helpful for the policy making.
