摘 要:目的 了解北京市中心城区社区65岁以上老年人静脉血常规情况,分析老年人静脉血血常规检测结果与年龄增长之间的关系. 方法 2011年2月至2011年9月,通过随机抽样方法选取北京市朝阳区潘家园社区、八里庄社区和西城区展览路社区、广外社区2014例老年人,通过问卷调查方式收集调查对象的基本资料、生活习惯、患病情况等信息,检测其血常规和生化结果.采用方差分析法分析血常规结果和年龄增长之间的关系. 结果 北京城区老年人的红细胞计数平均值为4.43×10^12/L,血红蛋白平均值为135.6 g/L,红细胞压积平均值为39.5%,白细胞平均值为6.69×10^9/L,血小板平均值为245.8×10^9/L;调查对象血细胞计数的95%可信区间分别为,红细胞计数(3.50~5.26)×10^12/L,血红蛋白106.6~164.6g/L,红细胞压积31.6%~47.5%,白细胞计数(3.26~10.00)×10^9/L,血小板计数(110.9~377.7)×10^9/L;男性红细胞计数、血红蛋白和红细胞压积均高于女性(P<0.05、P<0.001和P<0.001),女性血小板计数高于男性(P<0.001);随着年龄增长,红细胞计数、血红蛋白、红细胞压积和平均红细胞血红蛋白浓度(MCHC)逐渐下降(均P<0.001),红细胞平均分布宽度(RDW-CV)和单核细胞数呈递增趋势(均P<0.001). 结论 老年人血细胞计数的参考范围与成年人参考范围有所不同,是否需要单独制定老年人的参考范围还需要更多的数据分析;老年人血细胞计数指标随着年龄的增长呈动态变化,分析其变化原因并提出改善措施可以提高老年人的健康水平.Objective To investigate routine blood cell count among elderly people aged over 65 years,and to analyze the relationship between routine blood cell count and age.Methods 2014 elderly people were selected by randomly sampling methods from Panjiayuan community and Balizhuang community in Chaoyang District,Zhanlanlu community and Gungwai community in Beijing.Basic data,habits,illness and other information were collected by the survey questionnaires,and blood samples were drawn to analyze routine blood cell count testing and biochemical results.The relationship between routine blood cell count testing and age was analyzed by statistical analysis.Results In elderly people in Beijing community,the mean red blood cell count was 4.43 1012/L,the mean hemoglobin was 135.6 g/L,the mean hematocrit was 39.53%,the mean white blood cell count was 6.69 109/L,the mean platelet count was 245.8 109/L.The 95% confidence interval was (3.50-5.26) 1012/L for red blood cell count,was (106.6-164.6)g/L for hemoglobin,was 31.6%-47.5%for hematocrit,was (3.26-10.00) 109/L for white blood cell,was (110.9-377.7) 109/L for platelet.The red blood cell count,hemoglobin and hematocrit were higher in men than in women (P<0.05 or 0.001),and the platelet count was higher in women than in men (P<0.001).The red blood cell count,hemoglobin,hematocrit and mean corpuscular hemoglobin concentration (MCHC) were decreased along with ageing (all P<0.001),and the average red blood cell distribution width (RDWCV) and mononuclear cell count showed an increasing trend with ageing (both P<0.001).Conclusions The reference range of venous blood cells count in elderly people is different from that in adult people,and whether to develop a separate reference range for the elderly needs more data analysis.The values of blood cells count in elderly people may change dynamically with aging,and the elderly health can be improved by analyzing the reasons for the changes.
