摘　　要：目的:利用处方点评评价我院外科住院患者万古霉素/去甲万古霉素用药情况,提出行之有效的管理措施,促进万古霉素类药物的合理应用。方法:抽取我院外科系统2014年1月-3月使用万古霉素/去甲万古霉素的全部病例,依据《万古霉素/去甲万古霉素病历点评指南(试行)》进行处方点评,并对其数据进行统计分析。结果:共收集214份病例,无医生越权开药现象,67.3%的病例有专家会诊记录,手术预防用药占5.6%,治疗用药微生物标本送检率为55.4%,经验用药治疗有效率93.3%,目标用药治疗有效率100%,处方点评合理率为98.1%。结论:利用处方点评能够客观反映万古霉素/去甲万古霉素临床合理和管理情况,提示医院管理部门及时采取措施,促进合理用药。Objective： To evaluate vancomycin/norvancomycin management situation of surgical inpatients, propose effective management measures, and promote the rational use of vancomycin/ norvancomycin by prescription comment. Methods： From January to March, 2014, all surgical cases treated with vancomycin / norvancomycincases were selected in our hospital. The comment and statistical analysis were conducted on these cases, according to the ＆quot;Vancomycin/norvancomycin prescription comment guidelines （trial）＆quot;. Results： There were 214 cases with no unauthorizedly prescription of vancomycin/norvancomycin , 67.3% cases with expert consultation records, 5.6% cases with antibiotic prophylaxis for surgical procedures, 55.4% cases with microbial inspection, 93.3% cases with effective rate for experience in medicine, and 100% effective rate for target use. The reasonable rate of all these prescription was 98.1%. Conclusion： The use of prescription comment can objectively relfect the conditions of clinical use and control of vancomycin/norvancomycin, which reminds the hospital management department of taking steps to promote the rational use of vancomycin/norvancomycin.
