摘  要:目的:探究可溶性血栓调节蛋白(sTM)、高敏C反应蛋白(hs-CRP)血浆浓度与冠心病(CHD)的相关性。方法:根据冠状动脉(冠脉)造影结果,收集兰州大学第一医院心内科对照组(108例)、稳定型心绞痛组(SAP组,67例)和急性冠脉综合征组(ACS组,222例),采用酶联免疫吸附法(ELISA)检测sTM和hs-CRP血浆浓度,分析sTM和hs-CRP血浆浓度与冠脉病变范围的相关性和对冠心病的诊断价值。结果:各组sTM血浆浓度差异均有统计学意义,其中ACS组>SAP组>对照组[(97.31±4.19)pg/ml>(72.16±6.70)pg/ml>(52.29±2.28)pg/ml,均P<0.05];hs-CRP血浆浓度ACS组>对照组,且差异有统计学意义[(2.91±0.08)μg/ml>(2.55±0.09)μg/ml,P=0.012],对照组和SAP组、SAP组和ACS组差异均无统计学意义(均P>0.05);病例组hs-CRP和sTM血浆浓度与冠脉病变范围无相关性(均P>0.05);sTM和hs-CRP血浆浓度无相关性(r=-0.070,P=0.116);sTM和hs-CRP血浆浓度用于ACS的判断均有显著意义(P<0.001,P=0.048)。结论:sTM和hs-CRP血浆浓度均可预测冠心病患者的斑块稳定性,且对ACS有一定的诊断价值,但均与冠脉病变范围不相关。Objective:To investigate the association of high-sensitvity C-reactive protein(hs-CRP),soluble thrombomodulin(sTM)and coronary heart disease(CHD).Method:The 108 controls,67stable angina pectoris(SAP)and 222 ACS individuals from department of cardiology in the First Hospital of Lanzhou University were recruited by coronary angiography(CAG)in this study.This study was based on case-control study design.The ELISA was carried out to determine sTM and hs-CRP plasma levels.The differences of sTM and hs-CRP plasma levels in three groups and in the different range of coronary artery lesion were observed.The diagnosis value of sTM and hs-CRP plasma levels for CHD was also observed.Result:The three groups showed significant differences in sTM plasma levels each other[ACS group〉SAP group〉control group,(97.31±4.19)pg/ml〉(72.16±6.70)pg/ml〉(52.29±2.28)pg/ml,all P〈0.05];The plasma levels of hs-CRP were higher in ACS group compared with control group[(2.91±0.08)μg/ml〉(2.55±0.09)μg/ml,P=0.012],there were no statistical difference among the other interblocks(all P〉0.05);There were no statistical difference between sTM,hs-CRP plasma levels and the different range of coronary artery lesion(all P〉0.05).There was no correlation between sTM and hs-CRP plasma levels(r=-0.070,P=0.116).There were statistical significant for estimating ACS by sTM and hs-CRP plasma levels(P〈0.001,P=0.048).Conclusion:sTM and hs-CRP can predict the stability of coronary plaques in patients with CHD,and they may predict acute coronary syndrome,but they are not associated with the range of coronary artery lesion.
