2型糖尿病并发脑梗死的临床危险因素分析

摘　　要：目的分析2型糖尿病并发脑梗死的临床危险因素。方法选取确诊为2型糖尿病的患者508例,分为A组(并发脑梗死,n=240)和B组(无脑梗死,n=268)。记录2组患者的基本临床资料并检测其血糖、血脂、糖化血红蛋白(HbA1c)、血β2微球蛋白、尿β2微球蛋白、尿微量清蛋白、空腹胰岛素、空腹C肽等指标生化指标和凝血指标。结果 A组年龄、糖尿病发病年龄、高血压病程及收缩压与B组相比差异有统计学意义(P<0.05)。A组高血压及冠心病的发病率明显高于B组(P<0.05)。A组HbA1c、载脂蛋白B(ApoB)、空腹胰岛素、空腹C肽、脂蛋白a、同型半胱氨酸(Hcy)、血β2微球蛋白、尿β2微球蛋白、尿微量清蛋白与B组相比,差异均有统计学意义(P<0.05)。A组抗凝血酶Ⅲ水平明显低于B组(P<0.05)。结论年龄、高血压、HbA1c、ApoB、空腹胰岛素、空腹C肽、脂蛋白a、Hcy等是2型糖尿病患者并发脑梗死的危险因素。Objective To analyze the clinical risk factors of type 2diabetes complicated with cerebral infarction.Methods 508 patients with type 2diabetes were selected and divided into two groups：group A（complicated with cerebral infarction,n=240）and group B（without cerebral infarction,n=268）.The basic clinical information of two groups were recorded.The coagulation indicators and biochemical indicators（including blood glucose,blood lipid,HbA1 c,bloodβ2 microglobulin,urineβ2 microglobulin,urine microalbuminuria,fasting insulin,fasting C peptide etc）were detected respectively.Results There were significant differences in age,age at onset of diabetes,duration of hypertension and systolic blood pressure between group A and group B（P〈0.05）.The incidence of hypertension and coronary heart disease in group A were higher than group B（P〈0.05）.Comparing with group B,the levels of HbA1 c,ApoB,fasting insulin,fasting C-peptide,lipoprotein（a）,Hcy,bloodβ2microglobulin,urineβ2microglobulin,and urine microalbuminuria of group A were significantly different（P〈0.05）.The ATⅢ level of group A was significantly lower than that of group B（P〈0.05）.Conclusion The risk factors of type 2diabetes complicated with cerebral infarction include age,hypertension,HbA1 c,ApoB,fasting insulin,fasting C-peptide,lipoprotein（a）,Hcy,etc make diabetics be more prone to cerebral infarction ect.
