鼻内窥镜下双极电凝治疗老年性鼻出血89例临床体会

摘　　要：目的 探讨鼻内窥镜下双极电凝诊治老年性鼻出血的应用价值.方法 表麻下对89例老年性鼻出血患者（＞60岁）行鼻内窥镜检查,寻找出血点.有明确出血点者,行双极电凝治疗;对出血点不明确的,行鼻腔填塞治疗.结果 出血部位依次为：下鼻道穹窿顶44例,鼻中隔中后部23例,中鼻道7例,多部位5例,部位不明者10例.在鼻内窥镜下双极电凝止血79例,鼻腔填塞10例,全部病例1次痊愈,随访1～3个月无复发.结论 鼻内窥镜检查鼻出血视野清晰,双极电凝止血操作简单,病人痛苦小,止血效果好,值得推广.Objective to observe the application of bipolar electric coagulation in diagnosis and treatment of senile nosebleed under the guidance of nasal endoscopy. Methods 89 patients with senile nosebleed （〈60 years old） have been examined under nasal endoscopy： the patients who could be found out the bleeding area have been treated by bipolar electric coagulation find out the bleeding area, otherwise, the nasal cavity filling treatment would be applied. Results the bleeding area： 44 cases were in nasal vault top, 23 cases were in the back of the nasal septum, 7 cases were in middle nasal meatus, 5 cases were in multiple parts, 10 cases were unknown. Under nasal endoscopy, 79 cases have been treated by bipolar an coagulation, and 10 cases of nasal cavity filling, all cases have been healed just treating once, following-up visit for 1- 3 months without recurrence. Conclusion nasal endoscopic examine can find bleeding area clearly, and bipolar electric coagulation is a simple, less pain therapy.
