摘  要:目的探讨早期肠内营养对大鼠胃缺血-再灌注损伤(gastric ischemia-reperfusion injury,GI-RI)的影响及主要作用机制。方法建立大鼠GI-RI模型,将实验动物采用随机数字表法分为早期肠内营养组(early enteral nutrition,EEN)、单纯损伤组(I)和正常对照组(C),肉眼计数胃黏膜损伤指数、光镜下行病理评分、采用比色法检测胃黏膜脂质过氧化物丙二醛(maleic dialdehvde,MDA)含量和血清超氧化物歧化酶(superoxide dismutase,SOD)活力,并观察大鼠的体质量变化。结果 EEN组和I组的胃黏膜溃疡指数、病理评分、MDA含量、体质量下降值在GI-RI后明显高于对照组(P<0.05),SOD活力明显低于对照组(P<0.05)。EEN组和I组比较,除6h以外的各时间点,EEN组胃黏膜损伤指数、MDA含量、体质量下降值均明显低于I组(P<0.05),SOD活力高于I组(P<0.01)。72h病理评分EEN组低于I组(P<0.05)。结论早期肠内营养能明显减轻大鼠缺血-再灌注后的胃黏膜损伤,其可能机制是通过促进机体氧化与抗氧化平衡实现的。Objective To investigate the effects of early enteral nutrition on rats gastric ischemia reperfusion injury.Methods Setting GI-RI mode,and the rats were randomly divided into 3groups:early enteral nutrition group(EEN group),injury group(I group),and control group(C group).Gastric mucosal injury index,light microscope pathological score,gastric mucous membrane lipid peroxide MDA and vitality of serum superoxide dismutase was tested by using colorimetric method,and body mass change were observed.Results The gastric mucosal injury index,pathological score,the content of MDA,the value of weight loss in both EEN group and injury group were higher than those in control group after GI-RI(P〈0.05).Meanwhile,the activity of SOD were lower than control group(P〈0.05).The gastric mucosal injury index,the content of MDA,the value of weight loss exhibited significant decrease in EEN group compared with injury group at all time points except 6hafter the reperfusion(P〈0.05),and the activity of SOD was higher than injury group(P〈0.01).The pathological score was lower in EEN group than injury group at 72 hafter reperfusion(P〈0.05).Conclusion This study shows that early enteral nutrition exhibits a gastro protective effect against acute gastric mucosal lesions in rats induced by ischemia-reperfusion,which probably involves promoting the balance of peroxidation and antioxidation.
