基于关联规则挖掘名老中医王行宽肝心同治胸痹心痛用药规律

摘　　要：目的:挖掘名医王行宽辨治胸痹心痛的用药规律。方法:利用数码相机收集王行宽教授近1年来在湖南中医药大学第一附属医院门诊收治的胸痹心痛患者的医案267份,规范其中出现的中药术语,建立Excel数据库,运用Weka3.6软件,对医案中出现的药味进行描述性统计,采用Apriori算法对药味进行关联分析。结果:1用药讲究通补兼施,"通"予丹参、薤白、三七、柴胡、郁金、瓜蒌皮、法半夏等;"补"予白参、麦冬、白芍、茯神、炙远志、黄芪、茯苓等;2可信度≥0.90的药味关联多为丹参或柴胡与补气阴药、豁痰化瘀药之间的配伍;3可信度≥0.65的同类药味关联中,治心常用白参、五味子、白芍配麦冬,法半夏、黄连配瓜蒌皮等;治肝常予郁金配柴胡,天麻、白蒺藜配钩藤、石决明等。另外,王老治疗胸痹心痛"用药平和",药味几无峻烈之品,药量的运用亦多在常理之中。结论:"肝心同治、通补兼施、用药平和"乃王老临床辨治胸痹心痛的用药规律。Objective： To explore WANG Xingkuan＇s medication rule of chest Bi syndrome and heart pain. Methods：We collected Professor WANG Xingkuan＇s 267 patients with chest Bi syndrome and heart pain by using a digital camera in the outpatients of The First Affiliated Hospital of Hunan University of TCM in recent one year. Specification of the Chinese medicine and the establishment of excel database were carried out. Weka3. 6 software was used to make the descriptive statistics of medication. Apriori algorithm is adopted for the medication analysis of association rules. Results： 1The medication includes dredging and tonifying. Salvia,Allium macrostemon,Pseudo- ginseng,Bupleurum,Curcuma aromatica,Pericarpium trichosanthis and Pinellia ternate are for dredging and White ginseng,Ophiopogon japonicus,Radix Paeoniae Alba,Poria with hostwood,Polygala tenuifolia,Astragalus membranaceus and Poria cocos are for tonifying. 2 The credibility higher than 0. 90 medical combination is always the combination of Salvia and Bupleurum. 3The credibility higher than 0. 65 medical combination is always the combination of White Ginseng,Schisandra and white peony with Radix and combination of Berberine and Trichosanthes skin for treating heart. Treatment of liver always were Curcuma aromatica combined with Bupleurum,Gastrodia elata Blume and Tribulus terrestris combine with Rhynchophylla and Concha haliotidis,etc. In addition,WANG Xingkuan ＇s experience has no drastic ingredients. The use of dose is also in the common sense,not less or more. Conclusion： Treating liver- heart at the same time for dredging and tonifying is WANG Xingkuan＇s medication rule of chest Bi syndrome and heart pain.
