细胞因子表达水平与中医痰湿病理体质在阻塞性睡眠呼吸暂停低通气综合征中的相关性  被引量:9

Correlation between Cytokine Expression Levels and Phlegm Pathological Constitution in Obstructive Sleep Apnea Hypopnea Syndrome Patients

摘  要:目的:探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)中西医发病机制。方法:将医院收治的97例OSAHS患者分为痰湿体质组51例和非痰湿体质组46例,另选60例健康人群作为对照组;比较各组身体质量指数(BMI)、呼吸紊乱指数(AHI)、嗜睡指数(ESS)、血脂、细胞因子水平,并分析痰湿体质积分与各指标的相关性。结果:痰湿体质组BMI、AHI、ESS均显著高于非痰湿体质组和对照组;非痰湿体质组AHI明显高于对照组(P<0.05);痰湿体质组TG均显著高于非痰湿体质组和对照组,痰湿体质组和非痰湿体质组CHOL显著高于对照组(P<0.05)。痰湿体质组血清TNF-α、LP显著高于非痰湿体质组和对照组,APN显著低于非痰湿体质组和对照组,组间相比差异具有统计学意义(P<0.05)。相关性分析显示,OSAHS痰湿体质积分与BMI、AHI、ESS、TG、TNF-α呈正相关关系(P<0.05),与APN呈显著负相关关系(P<0.05)。结论:细胞因子异常表达、中医痰湿病理体质与阻塞性睡眠呼吸暂停低通气综合征的发病密切相关。Objective: To explore the western and TCM pathogenesis of obstructive sleep apnea hypopnea syndrome( OSAHS). Methods: 97 cases of OSAHS in our hospital were divided into 51 cases phlegm- dampness constitution group and 46 cases non phlegm- dampness constitution group. 60 cases of healthy people were selected as a control group.Body mass index of each group( BMI),respiratory disturbance index( AHI),sleepiness index( ESS),lipids,cytokines were compared and the physical phlegm integral correlation with the index was analyzed. Results: Phlegm- dampness group's BMI,AHI and ESS were significantly higher than those of the non phlegm- dampness and control groups. The non phlegm- dampness group's AHI was significantly higher( P〈0. 05). The phlegm- dampness group's TG was significantly higher than that of non physical phlegm group( P〈0. 05). Phlegm- dampness group's serum TNF- α and LP were significantly higher than that of the non phlegm- dampness and control groups and APN was significantly lower than that of the non phlegm- dampness group and control group and the difference between groups was statistically significant( P〈0. 05). Correlation: The analysis showed that OSAHS phlegm- dampness constitution's BMI,AHI,ESS,TG and TNF- α were positively correlated( P〈0. 05) and APN was a significant negative correlation( P〈0. 05). Conclusion:Abnormal expression of cytokines as well as phlegm- dampness pathological constitution is closely related to obstructive sleep apnea hypopnea syndrome pathogenesis.
