中年男性2型糖尿病患者胰岛素抵抗与前列腺体积的关系

摘　　要：目的：分析在中年男性新诊断的2型糖尿病患者中，胰岛素抵抗与前列腺体积（PV）的关系。方法40-65岁男性新诊断的2型糖尿病患者共193例，非糖尿病对照组57例，均检测空腹血糖（FPG）、空腹胰岛素浓度（FINS）、糖化血红蛋白（HbA1c）、血脂系列（TC、TG、LDL、HDL）及PV等相关临床指标，计算胰岛素抵抗指数（HOMA-IR）。结果糖尿病组的PV显著高于非糖尿病组（P〈0.05），糖尿病中胰岛素抵抗组（80例）PV明显高于非胰岛素抵抗组（113例）（t=2.740，P〈0.05）。良性前列腺增生（BPH）组（131例）的HOMA-IR显著高于非BPH组（62例）（t=3.320,P〈0.05）。相关分析显示，年龄、IR与PV呈正相关（r分别为0.263、0.309；均P〈0.05）。多元逐步回归分析显示，年龄、IR为PV的独立危险因素（b值分别为4.598、4.270；均P〈0.05）。结论2型糖尿病患者胰岛素抵抗是导致前列腺体积增加的危险因素。Objective To evaluate the relationship between insulin resistance and prostate volume（PV） in middle-aged male patients with newly diagnosed type 2 diabetes. Methods Totally 193 male patients from 40 to 65 years old and were included in this study.PV and biochemica1 analyses including fasting blood glucose,serum fasting levels of insulin（FINS）,HbA1C and lipid were performed.Insulin resistance index（HOMA-IR）was calculated.Results PV was more increased in diabetes group（193cases） compared with non-diabetes group （57cases） （t=2.441,P〈0.05）,and in insulin resistant group（80cases）compared with non-insulin resistant group （113cases） （t=2.740,P〈0.05）.HOMA-IR was more increased in BPH group （131 cases） compared with non-BPH group（62 cases）（t=3.320,P〈0.05）.PV was positively correlated with age and IR （r=0.263,0.309;P〈0.05）.Age and IR were independent risk factors of PV. Conclusions Insulin resistance in type 2 diabetes.might be the risk factors of increasing of the prostate volume.
