加味五味消毒饮治疗中重度痤疮疗效观察及对Th17细胞的影响

摘　　要：目的观察加味五味消毒饮治疗中重度痤疮疗效以及对Th17细胞的影响。方法检测健康对照组30名,轻度痤疮组32例以及中重度痤疮组45例外周血IL-17水平和Th17细胞在外周血中占CD4+细胞比例。45例中重度痤疮患者随机分成两组,治疗组采用加味五味消毒饮治疗,对照组采用异维A酸治疗,比较两组临床疗效和治疗前后Th17细胞相关指标。结果健康对照组、轻度痤疮组IL-17与Th17细胞两项指标显著低于中重度痤疮组(P<0.05),健康对照组和轻度痤疮组间差异无统计学意义(P>0.05)。中重度痤疮治疗组和对照组治疗后外周血Th17细胞相关指标显著降低(P<0.05)。结论加味五味消毒饮能有效治疗中重度痤疮,显著降低患者外周血IL-17水平和Th17细胞在外周血中占CD4+细胞比例。Objective To investigate the effect of Jiawei Wuweixiaodu decoction （JWD） on patients with moder-ate or severe acne vulgaris （MSAV） and its effect on Th17 cells. Methods Peripheral blood samples were drawn from 30 healthy controls, 32 patients with mild acne vulgaris, and 45 patients with MSAV to detect serum level of IL-17 and the proportion of Th17 cells. The 45 patients with MSAV were divided into two subgroups by random：treatment group（n=25） and control group（n=20）, receiving JWD and isotretinoin, respectively. The therapeutic effect and the level of IL-17 and the proportion of Th17 were compared between two groups. Results The serum level of IL-17 and the proportion of Th17 in healthy controls and mild acne vulgaris groups were lower than those in MSAV group（P〈0.05）, but no significant difference was found between healthy controls and patients with mild acne vulgaris （P〉0.05）. The total effective rate in treatment group and control group was 84% and 85%, respectively. The serum level of IL-17 and the proportion of Th17 were decreased after treatment compared with those before treatment in both two groups （P〈0.05）. Conclusion JWD can effectively treat patients with MSAV by lowering serum IL-17 level and the proportion of Th17 cells.
