单通道经皮肾镜下不同碎石术治疗鹿角形肾结石的疗效比较

摘　　要：目的评价单通道经皮肾镜超声碎石术与超声碎石联合输尿管软镜术治疗鹿角形肾结石的疗效。方法根据治疗方法不同,将65例鹿角形肾结石患者分为2组:单纯超声组31例行单通道经皮肾镜超声碎石术,联合组34例行超声碎石联合输尿管软镜术治疗,比较2组一期结石清除率、手术时间、出血量、术后住院时间、住院费用。结果联合组一期结石清除率高于单纯超声组(P<0.05),术后住院时间较短(P<0.05),住院费用减少(P<0.05),但手术时间较长(P<0.05),出血量比较差异无统计学意义(P>0.05)。结论单通道经皮肾镜超声碎石联合输尿管软镜术治疗鹿角形肾结石较单纯超声碎石术可提高一期结石清除率,缩短住院时间,降低费用,疗效较好。Objective It is to evaluate the clinical efficacy of single channel percutaneous nephrolithotomy （ PCNL）in ultrasound lithotripsy and the combination with ultrasound lithotripsy and flexible ureteroscope in the treatment of staghorn calculi .Methods According to their therapy method, 65 cases of patients with staghorn calculi were divided into two groups：simple ultrasound group with 31 cases were treated with the combination with ultrasound lithotripsy and flexible ureteroscope.Stonefree rate at one-stage, operation duration, amount of bleeding, postoperative hospital stay and hospitalization expenses were observed and compared in both groups.Results Compared with simple ultrasound group, stone-free rate at one-stage in combined group was higher （P〈0.05）, and postoperative hospital stay was shorter（P〈0.05）, and hospitalization expenses was lesser （P〈0.05）, but the operation duration was longer （P〉0.05）.The difference in amount of bleeding was not sta-tistically significant between the two groups （P＆gt;0.05）.Conclusion Single channel PCNL in combination with ultrasound lithotripsy and flexible ureteroscope ,with the advantages of high stone-free rate at one-stage and shorter postoperative hos-pital stay and lower hospitalization expensesin, is an ideal method for the treatment of staghorn calculi.
