左房增大合并房性早搏与阵发性房颤关系的探讨

摘　　要：目的探讨左房增大同时合并房性早搏与阵发性房颤的关系。方法根据心脏彩超及12导联动态心电图将120例房性早搏患者分为左房增大合房性早搏组(A组n=80例),无左房增大合并房性早搏组(B组n=40例)。对比两组阵发性房颤的发生率、及发生阵发性房颤患者中触发情况的比较。结果左房增大合并房性早搏组阵发性房颤发生显著高于无左房增大组,P<0.01(χ2=20.44);左房增大患者中由房性早搏触发阵发性房颤显著高于无左房增大组,P<0.05(χ2=4.083);由"P'on T"现象的房性早搏触发阵发性房颤发生显著高于非房性早搏触发的阵发性房颤。结论左房增大同时存在"P'on T"现象的房性早搏患者更易发生阵发性房颤,应早期药物干预降低阵发性房颤的发生率。Objective To investigate the relationship between paroxysmal atrial fibrillation and the coincidence of left atrial enlargement and atrial premature beats. Methods According to the results of echocardiography and 12 - lead Holter, 120 patients were divided into two groups, group A patients shown left atrial enlargement combined with atrial premature beats（ n = 80）, while group B did not show these phenomenon（ n = 40）. The incidence of paroxysmal atrial fibrillation and the triggering among these two groups were compared. Results The incidence of paroxysmal atrial fibrillation of group A was significantly higher than group B（P 〈 0. 01,X2 = 20.44）. And the incidence of paroxysmal atrial fibrillation, which was triggered by P - ON - T, is much higher than that was triggered by non - atrial premature beats. Conclusion The coincidence of left atrial enlargement and P - ON - T atrial premature beats is more dangerous for paroxysmal atrial fibrillation, and it should be prevented by drug treatment.
