摘  要:目的 颈动脉粥样硬化是导致缺血性脑卒中发生的主要原因之一,本研究对颈动脉超声(US)和CT血管造影(CTA)检测患者颈动脉狭窄的情况进行比较.方法 对2012年9月至2014年4月内蒙古医科大学附属医院神经外科94例已初步诊断颈动脉狭窄的住院患者,于入院后1周内作US和CTA检查,并对其中的5例患者同时行数字减影血管造影(DSA)检查.结果 二者对狭窄程度判断的一致性为74.17%.以DSA为金标准时,CTA正确诊断1例患者颈内动脉闭塞,但US低估为中度狭窄(50% ~69%).此外,US发现的16处斑块溃疡、斑块内出血及血栓形成,CTA未能显示.结论 CTA和US作为无创、简便、快捷检测颈部血管形态的技术,可以互补、联合应用诊断颅外颈动脉狭窄.Objective Carotid atherosclerosis is a major cause of ischemic stroke, the study of carotid artery ultrasound (DUS) and CT angiography (CTA) were compared between patients with carotid artery stenosis detection. Methods From September 2012 to April 2014: in Department of Neurosurgery, Affiliated Hospital of Inner Mongolia Medical University in 94 cases has been in the diagnosis of carotid artery stenosls m patients, DUS and CTA examination at 1 weeks after admission, and 5 cases of the patients with digital subtrac- tion angiography (DSA) examination. Results The consistency between the two stenosis degree was 74.17 %. With DSA as the gold standard, CTA correctly diagnosed 1 patients with internal carotid artery occlusion, but US underestimated moderate stenosis (50 - 69 % ). In addition, hemorrhage and thrombosis US found 16 plaque ulceration, intraplaque hemorrhage and thrombosis, CTA could not display. Conclusion CTA and US as a noninvasive, convenient, fast detection of neck vascular morphology, can be complementary, in diagnosis of carotid artery stenosis.
