摘 要:目的评价无创通气体外循环在婴幼儿拔除气管插管后呼吸衰竭中的临床应用效果,探讨其临床适用性。方法选择在该院儿科住院治疗的建立体外循环后发生呼吸衰竭患儿67例,简单随机分为观察组35例和对照组32例,对照组患儿给予气管插管辅助通气治疗,观察组则于术后采用无创通气治疗。观察二组患儿动脉血气指标的变化情况,使用呼吸机停机时间及二组患儿出现的并发症发生情况。结果二组患儿治疗后动脉血气指标均较治疗前明显改善,差异均有统计学意义(P<0.05),且观察组患儿各项指标的改善情况均优于对照组,差异均有统计学意义(P<0.05),观察组患儿辅助通气时间平均(55.58±14.72)h,对照组为(67.45±14.75)h,差异有统计学意义(t=3.2938,P=0.0016),观察组患儿并发症的发生率明显低于对照组,差异有统计学意义(χ2=6.1182,P=0.0134)。结论无创通气体外循环较气管插管辅助通气对呼吸衰竭患儿改善呼吸症状更合适,且并发症少,适合临床长期推广应用。Objective To evaluate the clinical effect of Non-invasive ventilation in infants with respiratory failure after extubation and to explore its clinical applicability.Methods A sample of 67 cases of pediatric respiratory failure in inpatient children was selected from March 2011 to September 2012 in the Affiliated Hospital of Hebei University,and they were randomly divided into two groups:the test group of 35 children and the control group of 32 cases.The control group received intubation assisted ventilation therapy,while the test group was given non-invasive ventilation support treatment.To observe the changes of arterial blood gas index,ventilator downtime and complications were recorded in the two groups.Results After treatment,the arterial blood gas indicators improved than before treatment,the differences had statistical significance(P〈0.05),and the improvement of the indicators in the test group was better than the control group(P〈0.05).The average ventilation time in the test group was(55.58±14.72)h,while the control group was(67.45±14.75)h,the difference had statistical significance(t=3.2938,P=0.0016).The incidence of complications in the test group was significantly lower than that in the control group,the difference had statistical significance(χ^2=6.1182,P=0.0134).Conclusions Non-invasive ventilation compared with intubation assisted ventilation can improve respiratory symptoms in children with respiratory failure,and is more appropriate and less complications.Non-invasive ventilation is suitable for longterm clinical application.
