摘  要:目的 良好的气道护理是安全有效人工气道治疗的基础。本研究的主要目的是通过随机对照设计比较微量注射泵持续和传统间断气道湿化护理对收住呼吸重症监护室(respiratory care unit,RCU)急性呼吸衰竭(acute respiratory failure,ARF)患者的有效性和安全性。方法 84例收住该院RCU的ARF患者随机平均分为二组,即持续湿化组和间断湿化组。持续湿化组中,使用微量注射泵持续向气管导管内注入湿化液5~10ml/h。间断湿化组中,手工注射湿化液3~5ml,1次/1~2h。每日由独立研究护士直接观察气道湿化效果、送检痰培养以及统计拔除气管导管时间和气道护理费用。结果 二组患者在基线特征方面差异无统计学意义。持续湿化组的日均吸痰量和次数明显低于间断湿化组,气道湿化良好的患者比例明显高于间断湿化组。二组患者痰培养阳性率差异无统计学意义。持续湿化组在拔管时间日均气道护理时间以及日均气道护理费用方面均明显低于间断湿化组。结论 应用微量泵可明显改善RCU中ARF患者的气道湿化效果,并减少气道护理负担。Objective To compare effectiveness and safety of persistent airway humidification(PAH)using microsyringe pump and intermittent airway humidification(IAH)among acute respiratory failure(ARF)patients hospitalized in respiratory care unit(RCU).Methods Eighty-four ARF patients hospitalized in RCU was randomly and equally divided into two groups,PAH group and IAH group.For PAH group,the humidification solution was injected into the tracheal catheter using the microsyringe pump at a flow rate of5~10mL/h.For IAH group,the solution was manually injected at a dose of 3~5mL at a time interval of1~2h.Independent research nurses were assigned to evaluate airway humidification outcome,deliver sputum microbial samples and assess time of extubation and airway care costs at a daily interval.Results The two groups were comparable in the baseline characteristics.PAH group had significantly less volume and time of daily sputum suction and a significantly greater frequency of good airway humidification outcome.The two groups were similar in positivity rate of sputum culture.PAH group had significantly shorter time of daily airway care time and smaller nursing costs than IAH group.Conclusions Use of microsyringe pump can improve airway humidification of ARF patients hospitalized in RCU and reduce airway care nursing costs.
