摘 要:目的观察雌激素及人重组生长激素对去势大鼠剩余牙槽骨骨保护素、破骨细胞分化因子(OPG和RANKL)和碱性磷酸酶(ALP)含量的影响。方法 4月龄未交配雌性SPF级去势骨质疏松大鼠65只,拔除左侧上颌磨牙建立联合剩余牙槽骨模型。建模成功后根据治疗方案分为5组:去势组(ovariectomized,OVX)、雌激素治疗组(estrogen replacement therapy,ERT)、生长激素治疗组(rh GH therapy,rh GHT)、联合治疗组(estrogen+rh GH therapy,Erh GHT)、假手术组(sham-ovariectomized rats,sham-OVX)。采用免疫组化的方法观察不同治疗组对剩余牙槽骨OPG、RANKL和ALP含量的影响。结果与shamOVX组相比,OVX组大鼠上颌剩余牙槽骨RANKL降低显著(P<0.05),而OPG表达略微增加,破骨细胞(OC)活性增强,剩余牙槽骨骨量减少。ERT、rh GHT和Erh GHT组均可升高OPG,降低RANKL,减弱OC活性,增加上颌剩余牙槽骨骨量(P<0.05)。去势后骨转换增强,血清ALP活性增强;ERT对血清ALP含量影响不大;rh GHT、Erh GHT可能通过增加成骨细胞活性,从而增强血清ALP活性。结论一定量的雌激素及人重组生长激素可保护去势大鼠剩余牙槽骨并增加成骨。Objective To explore effect of human recombinant growth hormone and estrogen on expression of OPG and RANKL on residual alveolar bone and ALP in serum of ovariectomized rats.Methods Sixty-five female 4-month unmated SPF ovariectomized rats were used to establish the residual alveolar bone model by extracting left maxillary molars of samples.Based on medication plans,the rats were divided into 5 groups:ovariectomized (OVX) group,sham-ovariectomized (sham-OVX) group,estrogen replacement therapy (ERT) group,rhGH therapy(rhGHT) group and estrogen and rhGH therapy(ErhGHT) group.Levels of OPG,RANKL and ALP were detected in all 5 groups using imnunohistochemical method.Results Compared with sham-OVX group,the expression of RANKL significantly decreased in OVX group,the expression of OPG increased slightly,the activity of osteoclast increased,and the residual alveolar bone decreased.Compared with sham-OVX and OVX groups,the expression of OPG elevated in ERT,rhGHT and ErhGHT groups,the expression of RANKL decreased,the activity of osteoclast reduced,and the bone quantity of residual alveolar bone significantly increased(P <0.05).The activity of ALP in serum significantly increased in rhGHT and ErGHT groups compared with shamOVX and ERT groups (P < 0.05).Conclusion Certain dose of estrogen and human recombinant growth hormone can improve osteogenesis of residual alveolar bone in ovariectomized rats.
