摘  要:目的 了解昆明地区新生儿葡萄糖-6-磷酸脱氢酶(G6PD)水平,以辅助筛查新生儿G6PD 缺乏症。方法 采集昆明地区29527例新生儿足跟血滤纸片干血斑标本,采用荧光分析法检测 G6PD 水平。采用SPSS13.0统计软件进行数据分析。结果 29527例滤纸片干血斑标本的G6PD水平为0.10~ 15.95 U/gHb,平均为(6.18±1.04)U/g Hb。男、女新生儿 G6PD 水平分别为(6.16 ±1.10)U/g Hb和(6.21 ±1.00)U/gHb,男女G6PD水平比较差异有统计学意义(t= -4.095,P 〈0.05);按不同采血年龄分为3组,分别为〈3 d采血组、3~7 d采血组和8~28 d采血组,其G6PD水平分别为(6.13±1.05)U/gHb、(6.20±1.03)U/gHb和(6.13±1.07)U/gHb。 〈3 d采血组和8~28 d采血组间 G6PD 水平比较差异无统计学意义(t=0.029,P 〉0.05),这2组G6PD水平与3~7 d组比较差异均有统计学意义(P均 〈0.05),且 G6PD 水平均值低于3~7 d组;按不同检测水平分为3组,分别为〈2.20 U/gHb组、2.20~2.60 U/gHb组和 〉2.60 U/gHb组。 〈2.20U/gHb组中男、女新生儿构成比与另外2组比较差异均有统计学意义(P均 〈0.05)。结论 昆明地区 G6PD缺乏症符合X伴性遗传方式,男性检出率高于女性;新生儿期任意时间采血对 G6PD 阳性检出率无影响;昆明地区G6PD缺乏症流行病学有其独特的特点,检出率有下降趋势。Objective To investigate the concentration of glucose-6-phosphate dehydrogenase (G6PD) concentration in newborns in Kunming areas for better assisting screen of G6PD deficiency among the live newborns. Methods A total of 29527 bloodspot filter paper specimens were collected from newborns in Kunming. The fluorescence analysis was used to detect G6PD concentration. All statistical analyses were conducted by using SPSS 13.0 software. Results The G6PD concentration of 29 527 bloodspot filter paper specimens was 0.10 - 15.95 U/g Hb,the average value was(6.18 ± 1.04) U/g Hb. The average G6PD concentration of male and female newborns were(6.16 ± 1.10) U/g Hb and (6.21 ± 1. 00) U/g Hb,respectively. The average G6PD enzyme concentration of female newborns was significantly higher than that of the male newborns( t = -4. 095,P 〈 0.05 ). The newborns were divided into three groups according to ages,the group of less than 3 days group,3 -7 days group and 8 -28 days group. The average G6PD enzyme concentration of 3 groups were(6.13 ± 1.05 ) U/g Hb, (6.20± 1.03 ) U/g Hb and( 6.13 ± 1,07 ) U/g Hb, respectively. There was no statistical difference between the less than 3 days group and 8 -28 days group in the average G6PD enzyme concentration( t = 0. 029,P 〉 0. 05 ) , but the average G6PD enzyme concentration of these 2 groups were significantly lower than that of 3 - 7 days group( all P 〈 0.05 ). According to concentrations, they were divied into 〈 2.20 U/g Hb group,2.20 -2.60 U/g Hb group and 〉2.60 U/g Hb group. The gender ratio in 〈2.20 U/g Hb group was statistically different between 2.20 -2.60 U/g Hb group and 〉 2.60 U/g Hb group( all P 〈 0. 05 ). Conclusions X-linked pattern of inheritance of G6PD deficiency is also observed in Kunming. The detection rate in male newborns is higher than that of female newborns.
