摘 要:目的采用静息态下低频振幅(ALFF)与功能连接的方法,观察长期使用海洛因对大脑相关脑区局部神经元的基线活动影响,并分析这些相关脑区神经活动改变与大脑功能组织模式的关系。方法采集17例海洛因依赖者和15例正常被试者的fMRI数据,分析海洛因依赖者ALFF的变化及其与使用海洛因的关系。以产生差异脑区为感兴趣区,用功能连接方法分析海洛因依赖者功能连接的改变。结果与对照组相比,海洛因依赖者的右侧尾状核、右背侧前扣带回、右额上回的ALFF明显降低,而双侧小脑、左颞上回和左枕上同ALFF增加。以上脑区中,仅右侧尾状核的ALFF值与海洛因使用总时间及每日剂量呈负相关。功能连接分析显示右侧尾状核和背外侧前额叶皮层(dlPFC)的功能连接降低,与小脑的功能连接增强。结论海洛因依赖者基线水平大脑神经元ALFF减少伴随着大脑功能连接的失调;慢性海洛因依赖者前额一纹状体环路和前额-小脑环路功能失凋,后者支持小脑内部模型理论以及促成成瘾者从机械行为向成瘾习惯的转换。Objective To explore heroin-induced damages on partial neural activities, and to analyze the relationship between these damages and the brain mode in functional organizationby observing its resting state amplitude of low-frequency fluctuation (ALFF) and functional connectivity. Methods Gathering fMRI scans of 17 male heroin dependent individuals and 15 equivalent normal subjects at their resting-state, using Rest and Dparsf software to analyze changes of ALFF in the brain regions of heroin dependent individuals; the relationship between ALFF changes with these individual's previous heroin consuming state was also analyzed. Then taking the differentiated brain parts as ROI, using functional connectivity method to analyze the changes of the connections of addiction-related alteration in the whole brain in the heroin dependent individuals. Results Compared with normal control group, the ALFF of heroin dependent individuals was reduced in the right caudate, right superior medial frontal cortex, and right dorsal anterior cingulate cortex (dACC), while increased in the left superior temporal gyrus, bilateral cerebellum, and left superior occipital gyrus. In these brain regions, only the value of the right caudate ALFF had a negative correlation with the overall time of heroin use. The functional connectivity between right caudate and dorsolateral prefrontal cortex (dlPFC) was reduced, while the functional connectivity between the right caudate and cerebellum was increased. Conclusion The reduction of ALFF and dysfunction of functional connectivity in heroin dependent individuals,and the dysfunction in the fronto-striatal and fronto-eerebell circuit,which may support the eerebellar internal model theory and contribute to the transformation from mechanical behaviors to addictive habits.
