256层CT冠状动脉成像对比剂用量应用研究

摘　　要：目的研究对比剂剂量对256层CT冠状动脉成像质量的影响。方法选择366例256层冠状动脉前瞻性心电门控扫描成像患者[体质量指数（BMI）〈30，心率50-80次／min]，将其按注射碘海醇350剂量分为3组，〈1.0mL／kg（105例）、1．0～1．1mL／kg（111例）及〉1．1～〈1．2mL／kg（150例）。由3位评估者以优、良、可、差4个等级分别观察不同对比剂剂量组及组内3种不同心率（≤60次／min、61～70次／min、71～80次／min）图像质量。结果3组不同注射对比剂剂量组间优秀图像、优秀良好图像差异无显著性（P〉0．05）。3组不同注射对比剂剂量组内的心率改变对图像质量影响有显著性差异（P〈0．05），低心率者（≤60次／min）图像质量优于心率较快者（71～80次／min）。结论正常BMI，正常心率者进行256层前瞻性心电门控扫描冠状动脉CT成像对比剂剂量（0．9～1．2mL／kg）不影响成像质量，心率是影响图像质量的重要因素。Objective To explore the influence of contrast agent medium volume on image quality of 256-slice coronary CT angiography. Methods 366 patients underwent 256-slice coronary CT angiography （BMI〈30,50-80 beat per minute） were included in the study,which were divided into three groups according to different volume of Omnipaque （350 mg I/mL）：〈1.0 mL/kg （n= 105 ）, 1.0- 1.1 mL/kg （n= 111 ） and 〉 1. 1- 〈1.2 mL/kg （ n= 150）. The images were reviewed by two independent radiologists and one nurse. The image qualities were determined in 3 contrast agent medium volume groups with different heart rate （ beast per minute） （≤60 beat per. minute, 61 -70 beat per minute,〉71-〈80 beat per minute） ,and 4 grades of the image qualities （the best, better,good and bad） were also assessed . Results There were no obvious significant differences in image quality among 3 contrast agent volume groups of coronary CT angiography （P〈0.05 ）. However, there were significant differences in image quality among different heart rates in every contrast volume group （P〈0.05）, and the image quality in patients with slow heart rate （≤60 beat per minute） was much better than that with fast heart rate （71-80 beat per minute）. Conclusion The contrast agent medium volume （0.9- 1.2 mL/kg） can not influence the image quality of 256-slice coronary CT angiography in patients with MBI 〈30 and heart rate 50-80,the heart rate can do.
