摘 要:目的:探讨苯妥英钠对华法林血药浓度的影响,为临床合理用药提供依据。方法:25只大鼠随机分为对照组及苯妥英钠2、5、8、11mg/kg组,每天上午给各组大鼠灌华法林,下午灌生理盐水或苯妥英钠,第2、4、6天分别取血样检测药效学指标凝血酶原时间(PT)和活化部分凝血活酶时间(APTT),第7天后取血样用高效液相色谱测定体内华法林的血药浓度。结果:2mg/kg组苯妥英钠与华法林对照组比较,华法林体内血药浓度无统计学意义(P>0.05),苯妥英钠5、8、11mg/kg组有统计学意义(P<0.05),苯妥英钠各剂量组组间比较,均具有统计学意义(P<0.05)。结论:给药浓度为5mg/kg的苯妥英钠可使大鼠体内华法林浓度上升,且随着苯妥英钠给药浓度的增大,抑凝血作用越强,呈现浓度依赖性。Objective:To explore effect of phenytoin on plasma concentration of warfarin.Methods:Sixty mice were randomly divided into five groups:control group and phenytoin 2mg/kg,5mg/kg,8mg/kg and 11mg/kg groups.All mice were given with orally warfarin in the morning,phenytoin or physiologic saline in the afternoon.The plasma concentration was determined after seven days with HPLC.Results:Compared to the control group,there was no significant difference in plasma warfarin concentration of phenytoin 2mg/kg group(P〉0.05),and the differences of phenytoin 5 mg/kg,8 mg/kg and 11mg/kg groups were significant(P〈0.05).There were significant differences among three phenytoin groups(P〈0.05).Conclusions:Phenytoin 5mg/kg can increase warfarin plasma concentration in mice.Furthermore,increase of hemagglutination inhibition by warfarin depends on the dosage of phenytoin.
