血清瘦素水平和冠心病患者危险分层及冠脉病变状况的关系

摘　　要：目的:观察接受冠状动脉造影患者的血清瘦素(Leptin)水平,探讨血清Leptin水平与冠心病危险分层及冠脉病变程度的相关性。方法 :将于我院做冠脉造影的患者150例,根据其临床表现及造影结果分为:非冠心病组35例,稳定性心绞痛(SAP)组37例,不稳定性心绞痛(UAP)组39例及心肌梗死(AMI)组39例,检测并分析各组患者血清Leptin水平,并分析其与冠心病危险分层相关危险因子的相关性。结果 :AMI组血清Leptin水平显著高于非冠心病组和SAP组、UAP组,差异有统计学意义(P<0.05)。患者血清Leptin水平随着冠脉损伤程度及Gensini积分的增加而增高,并与常用冠心病危险分层指标吸烟指数、超敏肌钙蛋白T(Hs-TnT)、超敏C反应蛋白(Hs-CRP)、氨基末端脑钠肽前体(NT-ProBNP)、低密度脂蛋白胆固醇(LDL-C)、Gensini评分呈正相关,与高密度脂蛋白胆固醇(HDLC)呈负相关。结论:血清leptin水平和冠心病患者危险分层相关因素密切相关,冠脉病变程度越重血清Leptin水平越高。Objective：To observe the level of serum leptin in patients with coronary angiography,and to analyze its relationship with the severity of coronary artery and its value in the coronary heart disease risk stratification.Methods：According to clinical manifestations and coronary angiography,150 cases in our hospital were enrolled and divided into 4group：non-CHD group（35cases）,stable angina pectoris（SAP）group（37cases）,unstable angina pectoris（UAP）group（39cases）and myocardial infarction（AMI）group（39cases）,respectively.The serum leptin concentrations in four groups were detected and relevance between serum leptin concentration with risk factors of coronary heart disease risk stratification coronary were analyzed.Results：Serum Leptin level in AMI group was significantly higher than that in the non-CHD group,SAP group and UAP group（P 〈0.05）.Leptin serum concentration showed the increasing trend with the increase of the coronary lesion severity and the i Gensini score.Leptin was positively correlated with the common indicators of coronary risk stratification smoking index,Hs-cTnT,Hs-CRP,NT-ProBNP,LDL-C and Gensini score,and was negatively correlated with HDL-C.Conclusions：Serum leptin has good correlation with usual risk factors of coronary heart disease risk stratification.The more severe lesion of coronary artery is,the higher level of serum leptin is.
