摘　　要：目的:探讨9 9mTc-DTPA肾动态显像在糖尿病肾病诊断中的临床价值。方法:120例2型糖尿病患者根据尿微量白蛋白分为大量白蛋白尿组(MAA组)、微量白蛋白尿组(MA组)以及正常白蛋白尿组(NA组),并选取40例健康人员作为对照组,分别检测4组人员身高、体重、体重指数(BMI)、血压、糖化血红蛋白(HbAlc)、血肌酐(Scr)及尿微量白蛋白,同时行9 9mTc-DTPA肾动态显像检查,并进行对比分析。结果:4组研究对象之间BMI比较无统计学差异(P>0.05);NA组糖尿病病程显著短于MA组,MA组显著短于MAA组(P<0.05);对照组收缩压(SBP)、舒张压(DBP)、HbAlc均显著低于3组糖尿病患者,同时MAA组显著高于MA组,MA组显著高于NA组(P<0.05)。对照组尿微量白蛋白、Scr及GFR均显著优于3组糖尿病患者;3组糖尿病患者中,Scr与GFR呈显著负相关(r=-0.186,P=0.008)。结论:9 9mTc-DTPA肾动态显像GFR能够较为准确地反映糖尿病患者肾功能状态,在糖尿病肾病的早期诊断或筛查中具有重要临床意义。Objective：To discuss the clinical value of 9 9mTc-DTPA renal dynamic imaging in the diagnosis of DN.Methods：A total of 120 patients with type 2diabetes mellitus（T2DM）were divided into the MAA group,the MA group,and the NA group based on urinary albumin,and 40 cases of healthy people were divided into the control group.Then height,weight,body mass index（BMI）,blood pressure,HbAlc,Scr,urinary albumin,and 9 9mTc-DTPA renal dynamic imaging were detected in the four groups and compared.Results：There was no significantly difference in BMI between the four groups P〉0.05）;Duration of T2 DM in the NA group was significantly longer than MA group（P〈0.01）,and MA group was significantly longer than MAA group（P〈0.01）;SBP,DBP,and HbAlc in the control group were significantly lower than the three groups of T2 DM patients（P〈0.05）,and MAA group was significantly higher than MA group（P〈0.01）,MA group was significantly higher than NA group（P〈0.01）;urinary albumin,Scr,and GFR in the control group were significantly better than the three groups of T2 DM patients（P〈0.05）.With the increase in urinary albumin,Scr was gradually increased,GFR was decreased in the three groups of T2 DM patients（P〈0.05）;and Scr and GFR was significantly negatively correlated.Conclusions：GFR detected by 9 9mTc-DTPA renal dynamic imaging can accurately reflect the state of renal function.It has important significance inthe early diagnosis or screening of DN in patients with T2 DM.
