血清孕酮与β-HCG联合检测在早期异位妊娠诊断中的预判价值

摘　　要：目的:探讨血清孕酮与绒毛膜促性腺激素β亚单位(β-HCG)联合检测在异位妊娠早期诊断中的预判价值。方法:选择北京航天总医院妇科病房收治的异位妊娠患者151例作为观察组,选择本院门诊同期常规正常早孕患者148例作为对照组,分别检测两组血清β-HCG、孕酮含量,评价两指标的预判价值,计算血清孕酮和β-HCG联合测定与单独β-HCG检测对异位妊娠诊断的灵敏度、特异度和准确率。结果:(1)异位妊娠组孕妇血清β-HCG、孕酮含量在孕龄30、40、50d均明显低于正常组孕妇血清β-HCG、孕酮含量,差异具有统计学意义(P<0.05);(2)血清孕酮与β-HCG联合检测对异位妊娠诊断的敏感性、特异性、诊断符合率明显高于β-HCG单项检测,差异具有统计学意义(P<0.05)。结论:血清孕酮与β-HCG联合检测相对于β-HCG单项检测有明显优势,可以弥补β-HCG单项检测的不足,对早期异位妊娠诊断有重要的临床指导意义。Objective：To explore serum progesterone andβ-HCG joint detection aredictive value in early diagnosis of ectopic pregnancy.Methods：A total of 151 cases of ectopic pregnancy patients treated in Department of Gynaecology,Beijing Aerospace General Hospital were selected as observation group,148 healthy pregnant women were selected as control group,serumβ-HCG,progesterone levels were detected.Diagnosis sensitivity,specific and accuracy of detecting serumβ-HCG level alone or combing with progesterone level detection were evaluated and compared.Results：（1）Serumβ-HCG,progesterone levels of the ectopic pregnancy group were significant lower than that of the control group 30 days,40days and 50 days of gestational age.（2）Combined detection of serumβ-HCG,progesterone levels showed significant higher sensitivity,specific and accuracy than detecting serumβ-HCG level（P〈0.05）.Conclusion：Combined detection of serum progesterone andβ-HCG levels show significant advantages comparing with detectingβ-HCG alone in diagnosing early ectopic pregnancy.
