双J管置入新方法在泌尿外科微创手术中的应用

摘　　要：目的 介绍一种简单有效的后腹腔镜下双J管置入新方法.方法 回顾性分析2010年1月至2013年12月本科室在微创手术下置入双J管的49例患者的临床资料,包括腹腔镜下输尿管切开取石术30例、腹腔镜下肾盂成形术12例、经下组肾盏经皮肾镜取石术7例.所有患者均使用输尿管导管在斑马导丝的配合下将双J管置入输尿管.记录术中双J管的置入时间;术后行腹部平片检查,记录双J管植入成功率及术中和术后并发症的发生情况.结果 49例患者术中双J管的置入时间为（3.0±1.0）min,成功率为100％ （49/49）,未出现双J管置入相关的并发症.结论 泌尿外科微创手术中利用输尿管导管在斑马导丝的配合下置入双J管,可以缩短手术时间,减少并发症的发生.Objective To delineate a simple and useful novel approach for placement of double-Jstent in retroperitoneal laparoscopic surgery.Methods A retrospectively analysis for the clinical profiles of 49 patients who underwent minimally invasive surgery in our department between January 2010 and December 2013 was performed,of whom 30 cases underwent laparoscopic ureter incision for extraction of calculi,12 laparoscopic pyeloplasty,and 7 percutaneous nephroscopy for extraction of calculi via the inferior renal calyces.Double-J-stent was placed with the use of ureteral catheter and zebra guiding wire.The duration of double-J-stent placement was recorded.Abdominal plain X-ray was assessed postoperatively.The success rate and the complication during and after the surgery were also documented.Results The average duration of double-J-stent placement was （3.0± 1.0） min,with the success rate being 100％ （49/49）.Stent placement-associated complications were not reported.Conclusion With the use of ureteral catheter and zebra guiding wire,the placement of double-J-stent is associated with shortened duration of surgery and lower incidence rate of complications
