间质性肺疾病中医证候与影像学特征临床观察

摘　　要：目的:前瞻性研究间质性肺疾病(ILD)中医证候的影像学表达特征及与证候的相关性。方法:211例ILD患者四诊资料、胸部HRCT报告作为观察内容。经中医辨证分为气阴两虚证、气阴两虚夹瘀证、气虚血瘀证、气虚痰瘀互结证、气虚痰热瘀阻证,分别研究不同中医证候的HRCT表现因子特征以及与影像学特征相关关系。结果:五种中医证候均以小叶间隔网格状、磨玻璃影病变为主。经统计学分析五种中医证型HRCT影像中结节状影、磨玻璃影、含气腔隙实变存在显著差别(P<0.05)。中医证候与HRCT影像学病变总积分之间可能存在线性相关。结论:五种中医证候分类具有一些HRCT影像学特点,推测证素组合越多,ILD影像学病变越重。Objective ： To prospectively research the traditional Chinese medicine syndromes of ILD and imaging expression characteristics and the relevance in syndromes. Method： 211 ILD patients＇ four diagnosis methods data and the report of chest HRCT were as the observation indexes. According to the syndrome differentiation, they were divided into syndrome of deficiency of both Qi and Yin, deficiency of both Qi and Yin and blood stasis, Qi deficiency and blood stasis, Qi deficiency and phlegm stagnation and blood Stasis, Qi deficiency and phlegm-heat stagnation and blood stasis. We studied difference of the syndromes in HRCT performance factors and the relevance in syndromes. Result： Five kinds of the syndromes in traditional Chinese medicine were all about septal grid and ground-glass opacities. According to the statistical analysis in the five kinds of syndromes of HRCT had significant differences on tubercle shadow and ground-glass opacities （ P〈0.05 ）. The traditional Chinese medicine syndromes of HRCT have a correlation with pathological changes. Conclusion ： Five kinds of traditional Chinese medicine syndromes have imaging characteristic. The more in speculate combination, the serious in ILD imaging disease.
