摘 要:目的观察氟康唑联合甲强龙体外诱导阿萨希毛孢子菌(Trichosporon asahii,T.asahii)获得氟康唑体外诱导耐药株的成功率及稳定性,为T.asahii唑类耐药机制研究奠定基础。方法对16株T.asahii进行氟康唑的真菌体外药敏试验,分别通过氟康唑、氟康唑联合甲强龙和单纯甲强龙对其进行体外诱导耐药,并与空白对照组进行比较。所有诱导获得的耐药菌株进行30次无药传代,每株菌在每次传代后均重新进行最小抑菌浓度(MIC)检测。结果空白对照组及单纯甲强龙组各菌株MIC值无明显变化;单纯氟康唑诱导获得耐药株12株(75%),氟康唑联合甲强龙诱导获得耐药株16株(100%);两组经30代传代后,单纯氟康唑组耐药株6株(37.5%),氟康唑联合甲强龙诱导获得耐药株9株(56.25%)。结论甲强龙可以提高氟康唑诱导T.asahii耐药株的成功率和稳定性,氟康唑联合甲强龙优于单纯氟康唑,更适用于T.asahii氟康唑体外耐药株的诱导。Objective To observe the in vitro induction successful rate and stability of fluconazoleresistant Trichosporon asahii(T.asahii) with fluconazole combined with methylprednisolone.Methods Sixteen strains of T.asahii underwent in vitro drug susceptibility test.The strains were induced with fluconazole alone,fluconazole combined with methylprednisolone,and methylprednisolone alone,respectively,and the drug resistance was compared with the control group.The resistant strains were passaged for 30 generations in drug-free medium,and the minimum inhibitory concentration(MIC) of each passage was detected.Results The control group and the methylprednisolone alone group had no obvious changes in the MIC during the induction.There were 12 resistant strains(75%) in the fluconazole alone group,and 16 strains(100%) in the fluconazole combined with methylprednisolone group.After 30 passages,there were 6 resistant strains(37.5%) in the fluconazole alone group,and 9 resistant strains(56.25%) in the fluconazole combined with methylprednisolone group.Conclusion Methylprednisolone is helpful for increasing the in vitro induction successful rate and stability of fluconazole-resistant T.asahii.Fluconazole combined with methylprednisolone is superior to fluconazole alone in construction of fluconazole-resistant T.asahii.
