摘 要:目的探讨外周血循环内皮祖细胞(EPCs)与2型糖尿病动脉粥样硬化(AS)的相关性。方法密度梯度离心分离80例2型糖尿病AS患者(糖尿病组)和40例健康体检者(对照组)外周血单个核细胞,计数并诱导培养获得EPCs;动态观察细胞生长情况并检测细胞的相关功能,流式细胞术检测细胞相关表面标志CDl33、CD31、CD34和血管内皮细胞生长因子受体2(VEGFR2)及循环EPCs数量。全自动生化分析仪检测血清三酰甘油、总胆固醇、空腹血糖等指标。全自动糖化血红蛋白分析仪检测糖化血红蛋白(HbAlc)水平。结果糖尿病组与对照组比较,外周血单个核细胞计数绝对值差异无统计学意义(P〉0.05);血清三酰甘油、总胆周醇、空腹血糖、HbAlc水平明显升高(P〈0.05),其他指标结果间差异无统计学意义(P〉0.05);诱导培养细胞至第10天出现典型的鹅卵石样改变,贴壁细胞的相关功能检测结果表明,糖尿病组诱导培养的EPCs黏附、增殖、血管新生能力及血管复杂程度均明显低于对照组。经流式细胞术检测细胞CD133、CD31、CD34和VEGFR2等特异表面抗原表达量分别为(98.90±4.26)%、(85.20±3.57)%、(3.90±0.44)%和(50.90±3.42)%;糖尿病患者循环EPCs数量与对照组比较[(78.6±15.8)比(113.6±53.2)个/mO显著降低(P%0.05)。相关性分析结果显示循环EPCs数量与Glu、HbAlc呈负相关(r=一0.596、一0.647,均P〈0.01)。结论长期高糖环境使得2型糖尿病AS患者循环EPCs数量降低,内皮功能受损,动脉硬化进程加快。Objective To discuss the correlation between endothelial progenitor cells (EPCs) and atheroselerosis in type 2 diabetes mellitus. Methods Peripheral blood mononuelear cells from 80 T2DM patients and 40 healthy people were collected by density gradient centrifugation. EPCs were counted and cultured. We observed cell growth dynamically and tested relative function of EPCs. Flowcytometric analysis was used to assay cells co-express surface markers (CD133, CD31, CD34, VEGFR2) and circulating EPCs. 7600-020E type automatic biochemistry analyzer was used to determine serum levels of TG (triglyceride), TC (total cholesterol) , LDL (low density lipoprotein) , Glu (glucose). TOSOH HLC-723G8 automatic glycosylated hemoglobin analyzer was used to test glycosylated hemoglobin (HbAlc). Results When comparing patients with T2DM to healthy subjects, there was no statistical significance of difference in peripheral blood mononuclear cells. Serum levels of TG, TC, Glu, HbAlc were higher in patients with T2DM than in heathy controls (P 〈0.05). Typical cobble-stone was present in cells until 10th day by inducing culture. The results of relative function test showed that cell adhesion, proliferation, vascular genesis and its multiplicity were obviously lower in group of type 2 diabetes mellitus than in controls. The expression levels of cell-specific surface antigen CD133, CD31, CD34 and VEGFR2 were (98.90 ± 4.26) %, (85.20±3.57)%, (3.90±0.44)% and (50.90±3.42)% respectively. Number of circulating EPCs were lower in patients with T2DM than in healthy subjects [(79.74±15.8)/ml vs. (113.6±53.2)/ml, P〈0.05~. The correlation analysis showed that circulating EPCs number had negative correlations with fasting glucose and HbAlc levels (r=--0. 596, --0. 647, respectively, all P〈0. 0001).
