摘  要:目的了解Th17相关的细胞因子IL-17,IL-4,IL-23在不同分期和分型的白癜风患者血清中的变化。方法选择2012年10月-2013年6月收集寻常型白癜风患者52例以及40例健康体检者的外周血标本,采用ELISA的方法检测血清中IL-17、IL-4和IL-23的水平。结果进展期、稳定期的白癜风患者血清中IL-17、IL-23水平均较正常对照组增高(P<0.05),而血清IL-4在三组之间差异无统计学意义;泛发型白癜风患者IL-17水平较正常对照组及局限型白癜风患者显著升高(P<0.05),局限型白癜风患者IL-17水平和正常对照组间差异无统计学意义(P>0.05);血清IL-23水平泛发型白癜风患者较正常对照组及局限型白癜风患者显著增高(P<0.05),局限型白癜风患者的IL-23水平较正常对照组也显著升高(P<0.05);而血清IL-4在三组之间差异无统计学意义。结论 IL-17及IL-23在进展期及泛发性白癜风患者血清中明显升高,提示其可能与白癜风发病有关。Objective To investigate the role of Thl7 cytokines IL - 17, IL -4 and IL -23 in patients with vitiligo. Methods Blood samples of 52 patients with vilitigo and 40 healthy controls were collected from October 2012 to June 2013. ELISA was applied to analyze level of IL-4, IL- 17, IL- 23 in peripheral blood plasma. Results The levels of IL - 17 and IL -23 in vitiligo patients at different stages and types were sig- nificantly higher than those in nomal controls, ( P 〈 0.05 ). However, the level of IL - 4 between vitiligo pa- tients in different stages and types and normal people showed no significant difference (P 〉 0.05 ). The level of IL- 17 in generalized vitiligo group was elevated compared to normal group and localized vitiligo group (P 〈0.05). However, localized vitiligo group displayed no difference with normal group in IL- 17 (P 〉 0. 05 ). The level of IL - 23 in generalized vitiligo group was elevated compared to normal group or localized vitiligo group ( P 〈 0.05 ), and its level in localized vitiligo group were significantly higher than that in nomal controls ( P 〈 0.05 ). However, the level of IL - 4 revealed no difference between three groups ( P 〉 0.05 ). Conclusion Serum IL - 17 levels were significantly increased in the vitiligo group compared with the control group due to the elevated IL - 23 induced Thl7 cell differentiation disorder. Therefore IL - 17 and IL - 23 play an important role in mechanism of vitiligo.
