钙蛋白酶对控制通气大鼠膈肌组织学的影响

摘  要:目的观察钙蛋白酶对控制通气膈肌组织形态学的影响,以探讨机械通气导致的膈肌功能异常可能的机制。方法 SD大鼠18只随机分为对照组、24h控制通气组(CMV组)和钙蛋白酶抑制剂组(calpeptin,CMVC组),观察各组大鼠膈肌组织学和超微结构的变化。结果 (1)光镜下显示CMVC组膈肌纤维横截面形状规则,毛细血管内皮略有肿胀,与CMV组相比,未见炎性细胞浸润和中央核的改变;CMVC组膈肌纤维异常组织和结缔组织显著低于CMV组(P<0.05)。(2)电镜下,与CMV组相比,CMVC组膈肌肌原纤维排列整齐,Z线清晰,未见线粒体空泡变;CMVC组膈肌纤维损伤密度显著低于CMV组(P<0.05)。结论钙蛋白酶抑制剂能够明显逆转控制通气所致的膈肌损伤,提示钙蛋白酶可促进通气所致膈肌损伤。Objective To clarify the mechanisms of calpain on morpbological properties of diaphragm fibers under controlled mechanical ventilation in rats. Methods group) were randomly divided into control group, 24 hours A total of 18 Sprague Dawley rats (6 in each of CMV group and treatment with 4mg/kg calpep- tin group (CMVC). Variants of diaphragm uhrastructure and light microscopic feature were observed. Results ( 1 ) There was a little swelling of capillary endothelial cells in CMVC by light microscopic. Abnormal via- ble, necrotic mass of inflammatory cells and swelling of capillary en,lothelial cells could not be observed. Compared with CMV, the proportion of abnormal muscle and connectiw~ tissue were significantly lower in CM- VC ( P 〈 0.05 ). (2) Sarcomeres from CMVC remained in register with adjacent sarcomeres and Z - band re- mained relatively intact in diaphragm, atrophic mitochondria could not be seen in CMVC under electron micro- graphs. Compared with CMV, the density of muscle injury was significantly lower in CMVC ( P 〈 0.05 ). Conclusion Calpeptin reversed the effects of CMV - induced diaphragm injury,it was that calpain contribu- ted the ventilator- induced diaphragm dysfunction.
