老年高血压患者血清胱抑素C与左室舒张功能的关系

摘　　要：目的探讨老年高血压患者血清胱抑素C(Cys C)水平变化与左室舒张功能的关系。方法回顾性分析老年高血压患者66例,依据其舒张功能分为正常组和减退组。采用乳胶免疫比浊法检测Cys C,彩色多普勒超声仪测量左室收缩及舒张功能参数,同时采集患者基础情况及血脂、血肌酐、尿素氮、尿酸等资料。结果和舒张功能正常组相比,舒张功能减退组患者Cys C升高,差异有统计学意义(P<0.05);肌酐、尿酸、尿素氮水平差异无统计学意义(P>0.05)。Cys C与左心室射血分数(LVEF)呈中度负相关(r=-0.617,P<0.05);以肌酐为控制变量的偏相关分析,Cys C与LVEF仍呈中度负相关(r=-0.591,P<0.05)。结论血清Cys C升高可能是老年高血压患者发生心功能不全的有效预测因子。Objective To investigate the relationship between serum cystatin C （Cys C）and left ventricular diastolic function in elderly patients with hypertension.Methods Retrospective analysis was performed in 66 cases of elderly patients with hypertension,who were divided into normal diastolic function group and diastolic dysfunction group.Serum levels of Cys C were detected by latex-enhanced immunoturbidimetric assay;the parameters of left ven-tricular systolic and diastolic function were measured by color doppler.The data such as the levels of lipids,creati-nine,urea nitrogen,uric acid and other information were also collected.Results Compared with the normal diastolic function group,the levels of serum Cys C were higher than that in diastolic dysfunction group（P 〈0.05）;the levels of serum creatinine,uric acid and urea nitrogen had no significant difference between two groups（P 〉0.05）.There was negative correlation between the level of Cys C and left ventricular ejection fraction （LVEF）（r =-0.61 7,P 〈0.05）;after controlled by creatinine,the partial correlation coefficient was -0.591 （P 〈0.05）.Conclusions In-creased serum Cys C may be an value predictor of heart dysfunction in the elderly patients with hypertension.
