抗菌药物人工骨对跟骨骨折患者术后感染治疗的临床研究

摘　　要：目的探讨抗菌药物人工骨治疗跟骨骨折术后感染伴钢板外露临床效果,以寻找最佳治疗方法,提高临床诊治水平。方法选取2010年4月-2013年4月70例跟骨骨折术后感染伴钢板外露患者为研究对象,将其分成两组,对照组35例,予以常规西医治疗,观察组35例,在对照组基础上加用抗菌药物人工骨治疗,观察两组治疗后临床效果差异性,采用SPSS16.0软件进行分析。结果治疗总有效率对照组为77.14%,观察组为91.43%;治疗6个月和18个月两组在Bohler角、Gissane角、跟骨高度上比较差异有统计学意义(P<0.05);观察组住院时间、病原菌消失时间、感染控制时间等均明显低于对照组,组间比较差异有统计学意义(P<0.05)。结论抗菌药物人工骨既能抑制病原菌,又能促进骨愈合,是跟骨骨折术后感染伴钢板外露良好方法。OBJECTIVE To investigate the clinical effect of antibiotic artificial bone for treatment of postoperative infection of calcaneal fracture with exposed steel plate in order to find the best treatment method to improve clinical diagnosis and treatment .METHODS Totally 70 patients with postoperative infection of calcaneal fracture compli‐cated with exposed steel plate during Apr .2010 to Apr .2013 were enrolled in the study and divided into two groups ,the control group of 35 patients treated with routine western medicine ,the observation group of 35 pa‐tients treated with antibiotic artificial bone plus the treatment in the control group .Differences in clinical effects after treatment were observed for the two groups .RESULTS The total efficiency was 77 .14% in control group and 91 .43% in the observation group .The differences in Bohler angle ,Gissane angle ,and calcaneal height were sta‐tistically significant between the two groups at 6 months and 18 months of treatment （P〈0 .05） .The hospitaliza‐tion time ,pathogen disappearance time and infection control time were significantly lower in the observation group than in the control group （P〈0 .05） .CONCLUSION Antibiotic artificial bone can inhibit pathogens and promote bone healing ,which is a good method for treating postoperative infection of calcaneal fracture complicated with ex‐posed steel plate .
