胸腔灌洗对开胸术后胸腔感染的临床治疗研究

摘　　要：目的探讨胸腔灌洗对开胸术后胸腔感染的临床效果,以期寻找最合适的方法,提高临床治疗水平。方法随机选取2010年4月-2013年4月70例开胸术后胸腔感染患者为研究对象,分成两组,对照组35例,予以传统的胸腔冲洗法,观察组35例,予以胸腔灌洗治疗,比较两组治疗后在临床效果、实验室检查等差异性,采用SPSS16.0进行软件分析。结果对照组治愈率为25.71%、总有效率为74.29%,观察组治愈率为45.71%、总有效率为91.43%,两组比较差异有统计学意义(P<0.05);两组在体温恢复正常时间、住院时间、症状改善时间、肺不张恢复时间、肺部啰音消失时间、生活质量评分和实验室指标白细胞、C-反应蛋白、肿瘤坏死因子、白介素-6两组比较差异有统计学意义(P<0.05)。结论胸腔灌洗治疗开胸术后胸腔感染临床效果显著,能缩短治疗时间,提高预后水平。OBJECTIVE To investigate the clinical results of pleural lavage for chest infection after thoracotomy in order to find the most appropriate way to improve the overall level of clinical treatment .METHODS Totally 70 cases of pleural infection in patients after thoracotomy in Apr .2010 to Apr .2013 were randomly selected for the study and divided into two groups ,the control group of 35 cases receiving traditional pleural flushing ,and the observation group of 35 cases receiving pleural lavage .Differences in the clinical effects and laboratory test results after treatment were compared .The software SPSS16 .0 was used for statistical analysis .RESULTS The cure rate was 25 .71% in the control group and 45 .71% in the observation group ,the total effective rate was 74 .29% and 91 .43% respectively ,the difference was significant （P〈0 .05） .The difference in time for body temperature to re‐turn normal ,hospital stay ,time for symptom improvement ,the recovery time of atelectasis ,disappeared time of pulmonary rales ,quality of life scores and laboratory parameters WBC ,C‐reactive protein ,TNF‐α,IL‐6 between the two groups was significant （P〈0 .05） .CONCLUSION The treatment of pleural lavage for chest infection after thoracotomy showed significant clinical efficacy ,which can shorten treatment time and improve the prognosis lev‐el .
