摘 要:目的 探讨血浆脂质运载蛋白型前列腺素D合成酶(L-PGDS)及内脂素水平与人股动脉粥样硬化并高血压及其相关危险因素的关系.方法 根据股动脉内膜-中层厚度(IMT)将152例患者分为股动脉粥样硬化组102例,健康对照组50例.股动脉粥样硬化组根据有无高血压分为股动脉粥样硬化并高血压组(48例),股动脉粥样硬化无高血压组(54例),测定空腹血浆L-PGDS与内脂素水平,所有受试者测定血压、身高、体质量、血脂、空腹血糖,计算体质量指数(BMI).结果 与对照组比较股AS患者血浆内脂素水平为(5.02±2.89) μg/L,明显偏高,血浆L-PGDS水平为(0.58±0.11) mg/L (P<0.05).股AS合并高血压患者与股AS患者比较,血浆内脂素水平[(28.89 ±4.33) μg/L]及L-PGDS水平[(0.90 ±0.30) mg/L]均明显升高(P<0.05).血浆内脂素与TG、LDL-C、CRP、BMI、IMT、DBP呈正相关,与年龄、HDL-C呈负相关(P<0.05),血浆L-PGDS与年龄、SBP呈正相关,与BMI、IMT及LDL-C、CRP水平呈负相关.血浆内脂素与L-PGDS水平呈负相关(P<0.05).结论 人股动脉粥样硬化并高血压患者血浆内脂素及L-PGDS水平明显改变,可能通过多种途径参与了动脉粥样硬化并高血压病的病理生理过程.Objective To investigate the correlation between the plasma levels of lipocalintype prostaglandin D synthase (L-PGDS) and visfatin with its risk factors in human fem-oral atherosclerosis with hypertension.Methods One hundred and fifty-two patients were detected by color Doppler with intimamedia thickness (IMT) measurement.According to IMT,the patients were divided into the femoral atherosclerosis group (n =102),and control group (n =50).The femoral atherosclerosis group was sub-divided into two subgroups:48 cases of femoral AS with hypertension (AS + H) and 54 cases of femoral AS without hypertension (AS).Fasting protein level of L-PGDS and visfatin in plasma,and blood pressure,height,weight,blood lipids,fasting glucose,body mass index (BMI) were measured.Results The level of serum visfatin [(5.02 ± 2.89) μg/L] was significantly increased in AS group as compared with control group,but the level of L-PGDS [(0.58 ± 0.11) mg/L] was significantly decreased in AS group (P 〈0.05).The levels of serum visfatin [(28.89 ±4.33) μg/L] and L-PGDS [(0.90 ±0.30) mg/L] were significantly increased in AS + H subgroup as compared with AS subgroup.Correlation analysis revealed that there was a negative correlation between plasma levels of visfatin and L-PGDS (P 〈 0.05).Plasma visfatin was positively correlated with TG,LDL-C,CRP,BMI,IMT and DBP,but negatively with age and HDL-C (P 〈 0.05).Plasma L-PGDS level was positively correlated with age and SBP,but negatively with BMI,IMT,LDL-C and CRP levels (P 〈 0.05).Conclusion The plasma levels of visfatin and L-PGDS significantly changed in hunan femoral artery atherosclerosis with hypertension,possibly through multiple pathways involved in atherosclerosis with hypertension pathophysiology of the disease.
