类风湿关节炎患者报告结局量表应用效果评价

摘　　要：目的在中医外治活动期类风湿关节炎(rheumatoid arthritis,RA)中评价类风湿关节炎患者报告结局(patient-reported outcome,PRO)量表的临床应用效果。方法将90例住院患者随机分为中药外敷组、扶他林组、中药离子导入组,经过2周的中药复方雷公藤外敷、扶他林乳胶外敷、中药离子导入治疗后,比较治疗前后及各组间血沉(ESR)、C-反应蛋白(CRP)及患者临床结局(PRO)量表评分。结果中药外敷组、扶他林组、中药离子导入组各组治疗前后血沉(ESR)、C-反应蛋白(CRP)及患者临床结局(PRO)量表评分差异有统计学意义(P<0.05),且中药外敷组治疗后在PRO量表中的反映躯体功能的第三因子方面与其他两组比较差异有统计学意义(P<0.05)。结论类风湿关节炎PRO能够较好地用于评价疾病情况,能从患者角度评估疾病整体情况,更为便捷,且可能比ESR及CRP具有更高敏感性。Objective To evaluate the effect of the clinical application of the patient-reported outcome（PRO） scale of rheumatoid arthritis（RA） at the active stage treated with the external therapy of traditional Chinese medicine（TCM）.Methods Ninety inpatients were randomized into a TCM external application group,a voltaren group and a TCM iontophoresis group.After 2 weeks treatment with the external application of compound tripterygium wilfordii,the external application of voltaren emulsion and TCM iontophoresis,ESR,CRP and PRO scale score were observed before and after treatment and among the groups.Results The differences were significant in ESR,CRP and PRO scale score before and after treatment among the TCM external application group,the voltaren group and the TCM iontophoresis group（P ＜ 0.05）.After treatment,in the TCM external application group,the score of the 3rd factor of somato-function in PRO scale was different significantly as compared with that in the other two groups （P ＜ 0.05）.Conclusion The PRO scale evaluates effectively the situation of RA as well as the overall condition of patients.This method is more convenient and possibly more sensitive as compared with ESR and CRP.
