摘  要:目的了解综合ICU呼吸机相关肺炎的发病情况。方法通过开展ICU目标性监测,调查2011年1月-2011年12月,入住综合ICU并接受呼吸机治疗的患者发生呼吸机相关性肺部感染的情况,并进行统计分析。结果 1 152名监测患者发生呼吸机相关性肺炎67例次,使用呼吸机总日数2 861 d,呼吸机使用率47.33%,呼吸机相关肺部感染率为24.52‰;共检出104株病原菌,其中,革兰阳性菌22株,占21.15%;革兰阴性菌72株,占69.23%;真菌10株,占9.62%。结论继续采取干预措施才能有效预防呼吸机相关性肺炎,治疗呼吸机相关肺炎应进一步加强对抗生素的合理应用的管理,减少耐药菌的发生。Objective The aim of the study was to assess the incidence of ventilator-associated pneumonia in general ICUs.Methods An ICU targeting surveillance was conducted to investigate the prevalence of ventilator-associated pulmonary infection among patients hospitalized in general ICUs and treated with ventilators between January and December of 2011.Results Among1152 monitored patients,67 cases of ventilator-associated pneumonia were reported.The patients used ventilators for a total of 2861 days,accounting for a utilization rate of 47.33% and a ventilator-associated pulmonary infection rate of 24.52 ‰.A total of 104 strains of pathogens were detected,including 22 strains of gram-positive bacteria(21.15%),72 strains of gram-negative bacteria(69.23%),and 10 strains of fungi(9.62%).Conclusion Only by continuing adopting intervening measures can ventilator-associated pneumonia be effectively prevented.Enhancing the management of rational antibiotics use should be emphasized in the treatment of ventilator-associated pneumonia to reduce the emergence of drug-resistant bacteria.
